During a routine inspection

We carried out this announced comprehensive inspection on 1 February 2023 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions.

We planned the inspection to check whether the registered practice was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations.

The inspection was led by a Care Quality Commission (CQC) inspector who was supported by a specialist dental advisor.

To get to the heart of patients’ experiences of care and treatment, we always ask the following 5 questions:

  • Is it safe?
  • Is it effective?
  • Is it caring?
  • Is it responsive to people’s needs?
  • Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

  • The dental clinic appeared clean and well-maintained.
  • The practice had infection control procedures which reflected published guidance.
  • Staff knew how to deal with medical emergencies. We noted that emergency medicines and life-saving equipment did not meet current guidance.
  • The practice had systems to manage risks for patients and staff, the risk assessments for fire and water systems needed review.
  • Safeguarding processes were in place and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • The practice had staff recruitment procedures which reflected current legislation.
  • Clinical staff provided patients’ care and treatment in line with current guidelines.
  • Staff took care to protect patients’ privacy and personal information.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • The appointment system worked efficiently to respond to patients’ needs.
  • The frequency of appointments was agreed between the orthodontist and the patient, giving due regard to the British Orthodontic Society guidelines.
  • There was effective leadership and a culture of continuous improvement.
  • Staff felt involved, supported and worked as a team.
  • Staff and patients were asked for feedback about the services provided.
  • Complaints were dealt with positively and efficiently.
  • The practice had information governance arrangements.

Background

Regent Orthodontics Limited is in Ilkley and provides NHS orthodontic dental care and treatment for adults and children.

There is step free access to the practice for people who use wheelchairs and those with pushchairs. Car parking spaces are available at the practice.

The practice has made reasonable adjustments to support patients with access requirements.

The dental team includes; 1 principal orthodontist, 5 orthodontic dental nurses,1 orthodontic therapist and a practice manager. The practice has 3 treatment rooms.

During the inspection we spoke with the principal orthodontist, orthodontist therapist, 3 orthodontic dental nurses and the practice manager. We looked at practice policies, procedures and other records to assess how the service is managed.

There were areas where the provider could make improvements. They should:

  • Take action to implement a Legionella risk assessment, taking into account the guidelines issued by the Department of Health in the Health Technical Memorandum 01-05: Decontamination in primary care dental practices. In particular, ensuring water temperatures are above the recommended temperature.
  • Implement an effective system of checks of medical emergency equipment and medicines taking into account the guidelines issued by the Resuscitation Council (UK) and the General Dental Council.

We carried out an announced comprehensive inspection on 7 November 2016 to ask the practice the following key questions; Are services safe, effective, caring, responsive and well-led?

We found that this practice was providing safe care in accordance with the relevant regulations.

We found that this practice was providing effective care in accordance with the relevant regulations.

We found that this practice was providing caring services in accordance with the relevant regulations.

Are services responsive?

We found that this practice was providing responsive care in accordance with the relevant regulations.

Are services well-led?

We found that this practice was providing well-led care in accordance with the relevant regulations.

Background

Regent Orthodontics Limited provides specialist NHS and private orthodontic care to children and adults. It consists of a team of orthodontists supported by an orthodontic therapist, a senior orthodontic nurse, five dental nurses, a practice manager and two receptionists. There is also a technician based at the practice.

The practice is based in the centre of Ilkley and has four treatment rooms, a decontamination room, office/ staff area, a technician’s workshop, a waiting room and reception area and a ground floor toilet. There is public parking available near the practice. The opening hours are Monday-Friday 8:30am-5:30pm, with the exception of Friday when the practice closes at 1pm.

The practice manager is the registered manager. A registered manager is a person who is registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the practice is run.

We received feedback from 60 people about the services provided. Patients commented that they found the staff friendly, caring and all the staff listened to them and took appropriate action. They commented that the orthodontists and therapist were knowledgeable and they were always given good and helpful explanations about orthodontic treatment. Patients commented that the practice was clean and comfortable.

Our key findings were:

  • The premises were visibly clean and free from clutter.
  • The practice had procedures in place to record and analyse significant events and incidents.
  • Staff had received safeguarding training, and knew the process to follow to raise concerns.
  • There were sufficient numbers of suitably qualified, skilled staff to meet the needs of patients.
  • Staff had been trained to deal with medical emergencies, and emergency medicines and equipment were available.
  • Patients’ needs were assessed, and care and treatment were delivered, in accordance with current legislation, standards, and guidance.
  • Patients received information about their care, proposed treatment, costs, benefits, and risks and were involved in making decisions about it.
  • Orthodontic treatment was well planned and provided in line with current guidelines.
  • Staff were supported to deliver effective care, and opportunities for training and learning were available.
  • Patients were treated with kindness, dignity, and respect.
  • The appointment system met the needs of patients, and emergency appointments were available.
  • Services were planned and delivered to meet the needs of patients, and reasonable adjustments were made to enable patients to receive their care and treatment.
  • The practice gathered the views of patients and took their views into account.
  • Staff were supervised, felt involved, and worked as a team.
  • Governance arrangements were in place for the smooth running of the practice, and for the delivery of high quality person centred care.

There were areas where the provider could make improvements and should:

  • Review the decontamination procedures and the monitoring of emergency drugs and equipment.  
  • Review the confidentiality and security of patients records stored in the reception area
  • Review the use of CCTV within the practice and fully comply with the Information Commissioner's Office (ICO) guidance.
  • Review the recruitment policy to ensure it reflects current practice.
