Safe
Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question requires improvement. At this assessment the rating has remained requires improvement. This meant some aspects of the service were not always safe and there was limited assurance about safety. There was an increased risk that people could be harmed.
This service scored 50 (out of 100) for this area. Find out what we look at when we assess this area and How we calculate these scores.
Learning culture
The provider did not always have a proactive and positive culture of safety. Records evidenced that historically when accidents and incidents occurred, it was not always clear what actions had been taken to reduce future risks. The service was in the process of implementing improvements to their processes, this included enhanced supervision of staff, moving records to an online system to allow real time oversight and reviewing and updating staff training. These improvements were not fully embedded at the service at the time of the assessment. Staff told us they were confident that when issues arose, these were dealt with quickly by managers. A staff member told us, “I know that if I raise any concerns, these are dealt with as quickly as possible.”
Safe systems, pathways and transitions
Safeguarding
The provider did not always work well to understand what being safe meant to them and how to achieve that. The service had had several changes in management over the proceeding months which had impacted on the availability of records within the service. Because of this we could not be assured that the correct notifications and referrals had always been made by the service in relation to incidents. A new management team was in the process of assessing staff training and knowledge and rolling out training which included training in safeguarding. Staff told us they had undertaken safeguarding training or were due to complete this soon. Staff told us they knew how to identify, and report safeguarding issues and felt confident the management team at the service would share this information with partners. People and their relatives told us they felt safe with the care staff who visited them.
Involving people to manage risks
Safe environments
The provider detected and controlled some potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care most of the time. The management team was in the process of implementing systems to monitor the safety of the equipment used for care delivery in people’s homes using online tools and care planning records. Not all records had been moved over to the new system at the time of the assessment. Staff told us that prior to the commencement of care an initial assessment is completed which included a review of the environment and any issues or safety concerns were shared with staff. However, care records did not always evidence that a robust assessment of the environment was completed prior to the commencement of care. Although planning and some actions had been completed to improve records and the assessment and monitoring of environments and equipment, this work was not fully completed and will take time to embed into practice. People told us they felt safe with the that was provided in their own homes. A person told us, “I feel safe with the carers, especially my regular carer.”
