During an inspection looking at part of the service

When we inspected this agency on 26 July 2012 we found the provider was non compliant with the following outcomes:- Respecting and involving people, Safeguarding people who use services from abuse, Requirements relating to workers, Assessing and monitoring the quality of the service provision and Notification of other incidents. The provider sent us their action plan which detailed how and by when they would achieve compliance with these essential standards of quality and safety.

This inspection was carried out on the 29 January 2013 in order to check whether the provider had taken action to achieve compliance. We found the provider had taken appropriate action.

We spoke with three service users and three relatives, both during the inspection visit and afterwards during telephone calls. We spoke with four care staff who were visiting the agency's office as well as two senior staff and the registered manager.

People using the service told us the times the staff arrived and the number of staff sent to provide their care had improved in the last two months and they were happy with the service they received.

Two relatives told us they were satisfied and happy with the care provided. One other relative told us that the staff's time keeping had improved but they remained disappointed with the lack of consistent care staff. Since the inspection visit this person had met with the registered manager, they told us this had reassured them and resolved their concerns.

During an inspection in response to concerns

We spoke with three people and one relative during telephone interviews as part of this inspection.

All the people we spoke with told us there were care plans, detailing the care they needed, kept in their homes. They said the staff kept these up to date and wrote in them daily, although they were unaware of what was written about them.

People said the staff generally treated them with respect and maintained their dignity during personal care.

Two people said the staff did not use their preferred names and they nearly always had to remind staff.

One relative told us the staff usually arrived on time and would let them know if they were delayed. Three other people said staff were very often late and they were not informed. One relative and one person using the service commented that up until a few months ago there were large numbers of different staff coming to their homes and they often didn't know who was coming. One relative said this had improved recently but three people said this was an aspect of the service that remained an issue.

People generally said staff were kind and patient and, where staff had not been so, they had told staff and this had been dealt with by the agency.

People did not always know how to complain but they said they would speak to staff they had got to know well if they had concerns.

Three people told us they felt staff managed their care safely and competently, one person agreed with the exception of the care provided by one member of staff who no longer delivered their care.

As part of this inspection we spoke to one member of care staff during the inspection visit at the provider's office and one member of care staff during a telephone interview after the inspection. We attempted to contact two more members of staff but they were unavailable.

During a routine inspection

During this inspection we spoke with six people who use the service and a relative.

People said they had been consulted about the care that they needed but four out of seven people we spoke with were not sure if they had received copies of care plans or if they had signed them. However six of the seven people told us that the staff did what they wanted them to do when they visited their homes. One person said "On the whole things are done". However other information we received for this inspection indicated that some instances of care had not been effectively provided such as when moving and handling people.

We received mixed views from people about the times of visits from staff. We also received information about some missed visits when a member of staff did not arrive to provide care. Three people also said that they did not always have the same member of staff so they did not always know their care needs. One person said 'the biggest problem was that there were so many different carers' visiting them.

Most people we spoke with were confident that action would be taken if they raised issues about their care. One person said they had raised a couple of issues and they had been acted on promptly by the agency. Another said their concern had not been resolved to their satisfaction.
