Person-centred Care

The provider aimed to deliver person-centred care, but care plans did not alwayscontainenough detail to guide staff unfamiliar with the individuals.While we found documentation was not always clear on how people were consulted, people felt the care they received was person centred. One person told us, “Staff always ask me what I want to do and respect my choices.”

Care provision, Integration and continuity

People experienced continuity of care and staff worked well with external professionals.The registered managerworkedin a collaborative way with involvedprofessionals.A person said, “They shadowed and made sure they knew everything and met me beforehand a few times to make the process a bit more comfortable.” This helped ensure smooth transitions and consistent care delivery.

Providing Information

Staff communicated clearly and adapted information to meet people’s needs. For example, the registered manager explained how verbal communication and family involvement were used for a person who could not respond to written documents, and plans are in place to introduce pictorial and large-print formats. People confirmed they were informed about changes and reviews.

The service promoted equality and diversity. Care plans recorded cultural and religious needs, and staff supported people to attend places of worship and celebrate cultural events. Forexample, Mealswere adapted to meet cultural preferences, and staffdemonstratedawareness of individual needs.

People reported positive experiences and outcomes. Feedback from surveys and compliments logs showed people were happy with the care and support they received.

Planning for the future

Score: 2

Planning for future care was limited. While one person’s end-of-life wishes were recorded, another person’s plan did not include this information. The registered manager acknowledged this and agreed to update care plans. There were no advance directives or clear contingency plans for hospital transitions, which could affect continuity of care during emergencies.
