Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People’s care and support needs were assessed before they started using the service to ensure the provider could meet their individual needs. Information gathered during the assessment process was used to develop person‑centred care and support plans, which outlined people’s needs, preferences and how they wished to be supported. A relative told us, “The managers came out and talked with us before it started.”

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. People remained independent at mealtimes, with staff support provided where required.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Staff worked effectively to ensure people’s needs were met. A staff member told us, “The best thing is the supportive team and the fact that we can see the positive difference our care makes to the people we support every day.”

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

The provider supported people to access healthcare professionals as required. When staff noticed changes in people’s needs their alerted the registered manager. A staff member told us, “I inform the manager quickly when such a situation arises. I also document it in people’s notes, so that everyone taking care of them will be aware and monitor them as well.”

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. The provider carried out regular assessments of the care provided, including spot checks of care delivery, and people were offered opportunities to provide feedback. A relative told us, “Yes, we see the manager regularly and talk through how things are going.”

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.Care plans include information about people’s mental capacity, their ability to consent or make decisions. Where required, mental capacity assessments were built into the electronic care system, guiding staff to complete Mental Capacity Assessments and record best interests discussions. Staff respected people’s rights. A staff member told us, “I always involve the person in decisions about their care. I ask them what they would like to do and respect their preferences, routines, and choices.”
