During a routine inspection

The inspection took place on 21 and 23 April 2015 and was unannounced. The home is a purpose-built, two-storey property set in its own grounds in a quiet residential area. There were 26 bedrooms on each floor, all of which had en-suite toilet, wash basin, and shower. On each floor there was a spacious lounge/dining room with a kitchenette, a quiet lounge and a bathroom. At the back of the house there was a well-tended garden.

The service is registered to provide accommodation and personal care for up to 52 people and 51 people were living there when we visited. The people accommodated were older people who required 24 hour support from staff. The home is part of the range of services provided by the Leeds based company Ideal Care Homes (North West) Limited and had a manager who was registered with the Care Quality Commission. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act and associated Regulations about how the service is run.

People we spoke with said they felt safe living at Upton Grange. All staff had received training about safeguarding and this was updated every year. There were enough qualified and experienced staff to meet people’s needs and keep them safe. The required checks had been carried out when new staff were recruited.

The staff we spoke with had good knowledge of the support needs of the people who lived at the home and had attended relevant training. The staff we met had a cheerful and caring manner and they treated people with respect. Both professional and family visitors who we spoke with expressed their satisfaction with the care provided.

We found that the home was well-maintained and records we looked at showed that the required health and safety checks were carried out. There was a lack of storage space for equipment which resulted in equipment being stored in bathrooms and wheelchairs being kept in the main entrance area.

We found that medicines were managed safely and records confirmed that people always received the medication prescribed by their doctor.

People we spoke with confirmed that they had choices in all aspects of daily living. They were happy with the standard of their meals and the social activities provided.

People were registered with local GP practices and had visits from health practitioners as needed. The care plans we looked at were comprehensive and gave details of people’s care needs and information about the person’s life and their preferences.

People we met during our visits spoke highly of the home manager. People were encouraged to complete satisfaction surveys and a programme of quality audits was in place to monitor the quality of the service.

During a routine inspection

During the inspection, we spoke with five people who use the service. They told us the staff explained what they were doing and always asked permission before delivering any care. They told us they were very happy living at the home and were treated well by the staff.

We found that people were asked for consent and the provider acted in accordance with people’s wishes. People who use the service received care in a way that met their needs and preferences.

We found the environment in the home to be clean, safe and well maintained. The people we spoke with told us they felt the home was clean and tidy and they felt safe. One person told us they were happy with the way the staff managed a recent outbreak at the home.

The people we spoke with told us the staff were helpful, nice and always did what was asked of them. They also told us they had no concerns about the care they received and were confident they could speak to the registered manager if they had any concerns.

We found that people were cared for by staff that had been through the appropriate recruitment checks. There was an effective complaints system available, in case anyone wished to raise a complaint.

During an inspection looking at part of the service

At our last inspection on 26 February 2013, we had identified concerns regarding how the service recorded information about people’s needs and how this information was reviewed.

At this inspection we looked at the care records of five people who used the service. We found that how the service recorded and reviewed people’s care needs had improved. However further work was needed to ensure the systems in place to audit care records was robust and identified issues of consistency in how information was recorded.

We spoke with two healthcare professionals who raised no issues of concern about how people were cared for and supported.

We observed people who used the service being involved in a range of activities during our visit including being entertained by a professional entertainer, going on an outing and preparing for a party. We observed members of the staff team supporting people in a respectful and supportive manner.

During a routine inspection

Records showed people who were considering moving into Upton Grange were given detailed information about the fees to be paid to use the service.

We observed care workers supporting people in a professional and supportive manner. People we spoke with made the following comments:

“I feel very lucky that I found this place, I am very happy here.”

“The staff are lovely and very kind, if I’m not well they always get the doctor for me.”

We spoke with one relative who told us told us they were very involved in how their relative was supported.

We observed care workers supporting people at a meal time. Care workers were respectful toward people who used the service and supported some people to eat their meal. We spoke with three people who told us they enjoyed the meals provided and were always offered an alternative if they did not like the meals offered. The dining areas were pleasantly decorated and the tables had been set prior to the next meal with tablecloths, napkins and cruet sets.

We were shown around the home and saw that all areas were pleasantly decorated and furnished and appeared clean and well-maintained. All parts of the building were warm.

We looked at the healthcare and nutritional care plans and risk assessments of four people. The level of detailed information held in these records varied and there was no consistency to how care workers recorded information.

During a routine inspection

People who use the service shared with us their feelings that they are happy with the care and support being provided and feel valued and listened to by the staff team working at Upton Grange.

The following information was gained through discussions with relatives/carers of people using the service, healthcare professionals and representatives of Wirral Local Authority Commissioning Team and observations made during the visit to the service:

'We looked around a few homes but found the facilities and the information offered about Upton Grange was the best'.

"This is a lovely home to visit'.

'I don't live here all the time I come when my family are away. I find the place good everyone is very helpful.

People who use the service, relatives and friends told us that the quality and variety of meals provided is mixed with some days meals being excellent and at other times poor.

They told us if they do not like the choices on offer they are always offered an alternative.

'Graham (manager) hasn't been here long but I feel if I was concerned about anything I would feel comfortable talking to him'.

'The staff are very caring and calm which I like'.

'I have no concerns about the care and supported being provided to patients here'.

'I'm happy here my family are made to feel welcome I like that'.

'I would talk to the staff if I was unhappy'.

Wirral local authority contracts and commissioning team told us they had no current concerns about the care and support being offered to people living at Upton Grange.

During the visit we heard and observed a member of staff engaging with a person who uses the service in an unsuitable and verbally abusive manner. We made the manager aware of this incident. He dealt with it appropriately and offered the person using the service reassurance and support.

We observed care workers and members of the management team supporting people who use the service in a respectful, sensitive and caring manner.
