During a routine inspection

We carried out an unannounced comprehensive inspection of this service on 24 and 25 June 2015. Breaches of legal requirements were found. This inspection took place on 6 and 9 May 2016.We found that the required improvements had been made to the safety and quality of the service and legal requirements were met.

Laburnums is registered to provide personal care and accommodation for up to 9 people who have a learning disability. There were six people receiving a service on the day of our inspection, including one person who was in hospital.

A registered manager was in post. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

Improvements were noted to the management of risks both for individual people and to the environment so as to ensure people’s safety. This included the safety of the premises and equipment used. Equipment such as that relating to fire or moving and handling equipment had been tested and checked to ensure it was safe and in good working order.

Systems to monitor and improve the quality and safety of the service were being effectively implemented. Regular checks were being completed by the staff, the registered manager and the provider’s representative to ensure action was taken where needed to improve the service.

Staff were knowledgeable about identifying abuse and how to report it to safeguard people. Recruitment procedures were thorough. Risk management plans were in place to support people to have as much independence as possible while keeping them safe. There were also processes in place to manage risks in relation to the running of the service.

Medicines were safely stored, recorded and administered in line with current guidance to ensure people received their prescribed medicines to meet their needs. People had support to access healthcare professionals and services. People had choices of food and drinks that supported their nutritional or health care needs and their personal preferences.

People were supported by skilled staff who knew them well and were available in sufficient numbers to meet people's needs effectively. People’s dignity and privacy was respected and they found the staff to be friendly and caring. People were supported to participate in social activities including community based outings.

Staff used their training effectively to support people. The manager understood and complied with the requirements of the Mental Capacity Act 2005 (MCA) and the associated Deprivation of Liberty Safeguards (DoLS). Staff were aware of their role in relation to MCA and DoLS and how to support people so as to ensure their rights were respected and met.

Care records were regularly reviewed and showed that the person had been involved in the planning of their care. They included people’s preferences and individual needs so that staff had clear information on how to give people the support that they needed. People told us that they received the care they required.

The service was well led; people living and working in the service knew the registered manager and found them to be approachable and available in the home. People had the opportunity to say how they felt about the home and the service it provided. Their views were listened to and actions were taken in response. The provider and registered manager had comprehensive systems in place to check on the quality and safety of the service provided and to put actions in place to improve it.

24 and 25 June 2015

During a routine inspection

This inspection took place on 24 and 25 June 2015 and was unannounced.

Laburnums is registered to provide accommodation with personal care for nine people who have a learning disability. There were nine people living at the home on the day of our inspection.

The manager in post was not yet registered. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run. The provider had not told us about these events as they were required to so we could see that they were taking suitable action to manage the service properly.

The provider and manager did not have robust systems in place to check on the quality and safety of the service provided, to put actions plans in place where needed, and to check that these were completed. You can see what action we told the provider to take at the back of the full version of the report.

Recruitment procedures were not shown to be thorough. Risk management plans were not always in place to keep people safe. Processes to manage any risks in relation to the running of the home were not established and effective.

Staff felt well supported and systems were in place to assess their competence. Training or updates were not always provided to staff promptly.

People were supported by staff who knew them well. There were sufficient numbers of staff to meet people's needs effectively. People’s dignity and privacy was respected. Staff were kind and caring in their approach to people. Visitors were welcomed and people were supported to maintain relationships and participate in social activities and outings.

Care plans were regularly reviewed and showed that the person, or where appropriate their relatives, had been involved. They included people’s preferences and individual needs so that staff had clear information on how to give people the care that they needed. People told us that they received the care they needed.

People knew the manager, found them to be approachable and to be improving the service. People living and working in the service had opportunity to say how they felt about the home and the service it provided. People felt confident that any concerns or complaints they may have would be listened and responded to.

23 October 2013

During a routine inspection

People had given their consent to care and treatment where they were able to and this had been regularly reviewed. Full pre-admission assessments had been carried out, taking into account people's diversity, cultural and religious needs and customs. Care plans were person centred and reflected people's individuality, choice and preferences. We saw that people had been offered choice in what they ate and drank. The daily food intake records showed that people had received a healthy balanced diet. People told us they were very happy with the food provided.

We saw that the service had good infection control policies, procedures and practices. Staff followed the Code of Practice for health and adult social care on the prevention and control of infections and related guidance. The environment was clean and tidy and generally well maintained. The provider needs to make improvements in their response to repairs as the home has been without an assisted bath since July 2013. Further repair work is needed in the same bathroom as we found broken, cracked tiles around the bath and flaking paint on the window ledge.

There were sufficient numbers of qualified, skilled and experienced staff to meet people's health, safety and welfare needs. People's concerns, complaints and comments were listened to and acted upon effectively. People received safe, effective and compassionate care from a well led service that responds quickly to their changing needs.

7 August 2012

During a routine inspection

People told us that they had regular meetings where they had discussed what they wanted to do and where they wanted to go. They said that they had used local transport to travel. One person said 'I go out most days and I am on my way out now to go the Salvation Army where I do gardening five days a week.' Another person said 'I go to my day centre every week where I do painting and crafts.'

People told us that they were often asked if they were happy with their care and support. One person said 'I am very happy here and I get the help that I need.' People told us that the staff were nice. One person said 'I am very happy with the way staff supports me to do the things that I want to do.' People told us that they enjoyed participating in activities of their choosing. One person said 'I enjoy painting and like having my work displayed around the home for others to see and enjoy.'

People said that they felt safe living at Laburnums. One person told us 'I am well treated and the staff look after me well, they help me to do what I need to do and I always feel safe with them.' People told us that the staff treated them well. One person said 'staff are always there to support me so that I can keep my independence as much as possible.' 'They all seem to know what they have to do to help me'. Another person said 'the staff are very kind and they help me a lot.'

People told us that they were happy with the quality of care provided at Laburnums. One person said 'It is nice living here, the food is good.' Another person said 'I like living here, I get out most days and I go for walks into Leigh, all of the staff treats me well.'
