During a routine inspection

Where people may have lacked the capacity to give their agreement, the home had not undertaken effective assessment to determine if people required more support to enable them to give informed consent.

Staff did not have access to clear guidance about the needs of people and how these needs were to be best managed.

We completed an audit of monies held on site and found balances to correspond to records held. However reconciliation of monies held had not happened despite an agreed action plan with CQC.

We spoke to people about the building and environment. Everyone we spoke to inlcuding staff, relatives and people living in the home said the home required work to make it safe. One person said, "I came to the Spinney because it used to have a good reputation; I wish I’d looked into things more before I’d moved in.”

We looked at training records of staff and noted that a real effort had been put into supporting staff with recent training. Staff who had recently started at the home described a suitable induction and had received formal supervision.

We looked closely at the audits undertaken. We found that whilst some areas of concern had been identified others had not. Concerns that had been identified for action were not always completed. Actions that had been completed were not done so in a timely manner

During a routine inspection

We spoke at length with six people who lived in the home and three relatives. They all said staff were working very hard in what they thought to be difficult circumstances due to a lack of staff. One relative said, 'Staff are so busy but they are always courteous and show (family member) respect.'

Staff we spoke too knew the needs of the people living in the home. Some staff we spoke to felt that at times people's needs were not met in a timely manner.

There were not effective systems in place to manage all different floats of money held. This left a potential risk of financial abuse.

People we spoke with who lived in the home thought that access to the bathrooms was a problem. One said,'Not only do I have to wait for ages to be taken for a bath; I have to go to the other end of the house.'

Relatives we spoke with said their family members appeared happy and liked the staff. One member of staff said, 'We eventually get everything done that we need to but we don't get enough time to just spend time with residents.'

Some relatives we spoke with had taken part in surveys and said that they felt comfortable raising issues or concerns.

During a routine inspection

It was clear from talking to people who live at the home that they are happy with the support they receive from caring, competent staff. Comments from people who live at the home included, 'All the girls are caring people'. Also, 'Very helpful and polite'. We spoke to staff and watched throughout the day routines being followed to the wishes of the residents and caring staff supporting people in the lounge area, ensuring they are well looked after.

During the visit we watched staff showing people respect, dignity and privacy by helping with care needs and meals in a sensitive manner. Staff were also observed knocking on doors and talking to people with respect. One person spoke about the attitude of staff and said, "Nothing is to much trouble".
