Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

Records showed staff reported incidents or accidents promptly and we saw records that supported this. There were systems and processes that recorded accidents, incidents and near misses that ensured any required changes in a person’s needs would be identified and additional safety measures put in place where needed.

The provider understood their responsibilities under Duty of Candour and relatives confirmed they were kept informed of any incidents.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

Care plans ensured staff had access to guidance on how to support people safely.Staff commented positively on the care plans used to support people. Care records showed where healthcare partners had been involved in people’s care and staff were instructed within the care plans to follow this professional guidance.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect.

Staff were able to clearly describe to us how they would identify potential abuse and what action they would take to escalate this appropriately. One staff member told us, “I have been trained in safeguarding, I recognise signs of abuse like unexplained bruises, cuts, the persons mood suddenly changing, becoming withdrawn. There are lots. Any of these signs I need to escalate to my manager immediately.”

Relatives were positive about the safety within the service. One relative said, “My relative and I am very happy with the care. They are an excellent company. The care is first class.”

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

Care plans were detailed with information for staff on how to avoid potential triggers in relation to known risks. People had been actively involved in managing risks to their health, safety, and wellbeing in ways that supported their independence and respected their individual preferences. Risk assessments were completed for people’s care and support needs.

Incidents recorded by staff were analysed by the provider and senior staff to ensure necessary actions had been taken to mitigate against risk. This ensured relevant organisations were notified when required.

Safe environments

Score: 3

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

Environmental risk assessments had been completed and formed part of the person’s care plan. Risk assessments included who was at risk and measures to take to reduce the risk. Care plans identified where healthcare professionals such as occupational therapists gave advice on how to support people in their home environment.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. Staff told us they worked together well to provide safe care that met people’s individual needs.

Staff were recruited safely with appropriate checks taking place, they took part in an induction and training programme to ensure they were competent and had the right skills to provide safe care.Supervision and appraisal processes were in place.

Staff spoke positively of the induction and training they had received from the provider stating, “Induction was training online. Moving and handling face to face. Maybe 5 shadow shifts. Every year the training has been updated. This made me feel confident. The annual refreshers are very helpful and keep me up to date with best practices.”

People and their relatives told us they felt safe, and the staff were well trained. One relative told us, "The carers are all very well trained and effective at what they do. I also find the company organisation and manager very effective.”

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading.

Staff had received training in infection prevention and control, to support their understanding of best practice. Staff reported there were sufficient supplies of gloves, aprons, and other personal protective equipment (PPE) available. No concerns were raised by people or their relatives when we spoke with them about infection control practice by staff.

Medicines optimisation

The provider made sure medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people and their relatives in planning where appropriate, including when changes happened.

Care records highlighted how medicines were stored, administered, recorded and key information such as allergies was recorded. There were processes in place to ensure people received their medicines in line with their prescriptions. Regular quality assurance checks were carried out. No concerns were raised by people or their relatives around medicines management.

Staff received training in medicines management and had their competency regularly assessed to support safe practices. Staff understood the importance of administering medicines at the prescribed times.One told us, “I am trained to administer medications. I check the care plan and the medication containers to check dates and quantities to ensure safety. I gain consent from client and administer in line with training. I record on the app with my signature and time. If I had any concerns about meds I would escalate this immediately to the manager.”
