Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People’s needs, wishes and preferences were identified before admission through a detailed assessment process. This included their health, wellbeing, personal care and social needs. This information was used to develop care plans, helping to ensure care was person-led from the start. This process was completed by the registered manager face to face and included information sharing about the service, including the service user guide.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.

The service supported people in line with legislation and used current evidence based good practice effectively. Care records provided evidence to show good practice guidance was followed in areas such as skin integrity.

How staff, teams and services work together

The provider worked well across teams and services to support people.

There was evidence of good team working within the service and with external partners. There were effective links with families and other health and social care professionals. Staff felt supported and included in the day to day running of the service. There were regular team meetings, and staff told us they felt involved and able to share their views. Feedback from allied professionals working with the service was positive. Comments included, “The carers who come are consistently professional, compassionate, and reliable. They treat everyone with dignity and respect, always taking the time to listen and understand individual needs."

 

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

Systems were in place to ensure people had access to specialist support and professional advice was used to inform care plans.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

Processes were in place to evidence people’s care and support was monitored. For example, where required, people’s skin integrity and food intake was monitored.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

Processes were in place to assess people’s mental capacity. People were routinely offered choice and control over how they wanted to live their lives. Staff demonstrated respect for the choices peoples made. Comments included, “I give choices all the time. People decide themselves.”
