This is the first assessment for this newly registered service. This key question has been rated good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

There were processes in place to report incidents and accidents, and we viewed some lessons learnt. We spoke with staff about reporting incidents and accidents and staff were knowledgeable about the correct procedures. Staff told us, "We have an incident policy in place.” Staff also told us, "If I had concerns about a person's safety, care, safeguarding, or staffing, I would report them immediately through the right channels. Currently, no concerns, if any I would discuss any concerns openly with management to ensure they are addressed."

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

The registered manager told us how they carried out pre-assessments before they started new care packages for people. Staff told us, "There's always assessments carried out if people go into hospital or have a fall, we carry out another assessment or update it.”

One relative told us, "Yes we had an interview with the [registered manager], I introduced them to [name] and we discussed their needs at the time and needs in terms of visits. The [registered manager] was really helpful.”

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns appropriately.

Staff we spoke to understood safeguarding and how to keep people safe. Staff had received the relevant training. Staff told us what they would do in the event of an incident, "I ensure the person is safe, seek medical assistance if needed, inform management, complete an incident or accident form, and record all relevant details."

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

Risk assessments were in place for identified needs, and these were detailed in people’s care plans. Staff told us, "Each person has individual risk assessments covering their home environment, mobility, medication, health needs. We follow these and report any new risks identified."

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. Risk assessments of people’s homes and environment were detailed in their care plans. This included any equipment they use, such as hoists and hospital beds.

Safe and effective staffing

Score: 1

The provider did not make sure there were enough qualified, skilled and experienced staff. They did not always make sure staff received effective support, supervision and development. Recruitment processes were in place, however we found some gaps in people's recruitment files. External people were supporting the service with supervising staff and signing off staff competency checks, however they had not been properly onboarded with the agency. They had not completed safe recruitment processes such as completing their General Data Protection Regulation (GDPR) training and not had Disclosure and Barring Service (DBS) checks, or that they were suitably qualified to undertake these responsibilities. Since we raised this with the provider, they have taken measures to ensure all people working for the service are safely recruited.

Following the inspection the registered manager has been proactive in improving the recruitment process to ensure safe and effective staffing. They had also instructed an agency to cover staff sickness to ensure people were able to get the care and support they needed, despite any potential staffing issues.

Infection prevention and control

Score: 3

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. Infection prevention and control (IPC) policies were in place and staff had access to the appropriate personal protective equipment (PPE). Staff had training to reduce the spread of infection.

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. Staff had received training to administer people’s medicines safely and as prescribed. Staff told us, "I have completed medication training, and I was observed to ensure I follow the 6 Rights of medication administration; the right person, right medication, right dose, right route, right time, and right documentation."

Relatives told us how the service worked in collaboration with the hospice team around managing pain for people.

One relative said, "Yes they are really good at administering medication, they log all medicines, so they are accountable."
