Care provision, Integration and continuity
The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity.
Systems and processes were in place to support continuity of care and to ensure people had access to relevant health professionals when they needed them. Staff understood people’s needs and supported them in attending appointments when required.

Equity in experiences and outcomes
Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.
People were given opportunities and choices to positively impact their experiences. The management team and staff focused on people’s quality of life, ensuring they had the same opportunities as people who did not use services. For example, accessing appropriate health care, engaging in the community, participating in social activities and maintaining relationships with family and friends. Feedback was consistently positive about people’s experience. Comments included, “What they do at Thomas Owen House is really, really good. It gives my [relative] life it's really important” and “[Name] thinks of it as home. [They have] never called anywhere home before they went to Thomas Owen House. Our experience is positive. It's a positive place for [Name].”
