Supporting people to live healthier lives
The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.
People’s physical health was monitored and referrals made to other health professionals when needed. For example, the GP and dietitian. Professional advice was documented in people’s records to support their health needs. Staff supported people’s choice and independence. Feedback included, “Independence is promoted, and people’s choices are supported always” and “People are reluctant sometimes, but we prompt them to be independent. We motivate them. We don’t force people to do things. They have a choice.” People were given a choice of activities to suit their own individual wishes and needs, to enhance their wellbeing.
