Person-centred Care

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs. At this inspection we found that people’s care plans had been reviewed and updated, they were now more reflective of their full physical, mental, emotional and social needs. Care plans were more detailed, up to date and provided clearer guidance to support personalised care. People told us they were involved in discussions about their care, and relatives also told us they were informed and included in the process. We saw several examples of how people were supported to pursue their hobbies and interests such as going to the gym and keeping up to date with the football, this demonstrated a person-centred approach to supporting people’s wellbeing.

Care provision, Integration and continuity

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. At this inspection we found improvements in how the provider understood and responded to people’s health and care needs. People’s care and treatment were now more aligned with their assessed needs, and we saw evidence that recommendations from external health and care professionals was being followed. Staff made timely referrals to agencies such as speech and language therapy and occupational therapy, which supported people to receive appropriate support when their needs changed.

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs. At this inspection we found improvements in how information was shared with people and their relatives. People told us they had more opportunities to be involved in their care planning, and we found a keyworker system in place to support regular communication and reviews. Relatives also told us there were updates and felt included in discussions about their family member’s care. One relative told us, “Tapton Grove have been good with communicating and they do the very best they can with the resources they have”.

Listening to and involving people

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result. People and staff told us they had opportunities to share their feedback and knew how to raise concerns. We reviewed a recent concern the service had received and found it had been investigated and responded to appropriately. People and their relatives told us they knew how to raise complaints and felt confident that, if they did so, the provider would work with them to resolve any issues.

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it. We found people were able to access care, support and treatment, both in and out of their home, when it was required. Staff knew people well, and people told us they felt staff understood their needs. Care plans contained clear information on when staff should escalate concerns and the signs to look for if a person’s health was deteriorating. This helped ensure people received timely support and that any changes in their health were recognised and acted upon promptly.

Planning for the future

Score: 3

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life. We found the provider had made improvements in how they supported and recorded people’s plans for their future. Care plans clearly set out the goals people wished to achieve and how staff should support them to work towards these. We also found that where people had shared their wishes for care at the end of their lives, this was recorded sensitively and appropriately, so staff were aware of what was important to them and could act in line with their preferences.
