During an inspection looking at part of the service

Acorn provides accommodation and personal care for 6 people. There were five young men living at the home when we inspected. This was an unannounced inspection, which meant the staff and provider did not know we would be visiting.

There was a registered manager in post. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

People were receiving care that was effective and responsive to their changing needs. Care plans were in place that described how the person would like to be supported and these were kept under review. People were involved in making day to day decisions and decisions about their care. Other health and social care professionals were involved with regular meetings taking place with the person and the staff supporting them.

People led very active lifestyles based on their interests and aspirations. Some people were supported in these activities whilst others were fully independent. Staff were knowledgeable about the risks to the person and others and what measures were in place to ensure the person’s safety. People were involved in discussions about how they could keep safe. Staff clearly described what they would do if they felt a person was a risk of abuse and the importance of reporting to other agencies.

People were receiving medicines in a safe way. Staff had implemented some new recording to improve how medicines were checked and ensured some medicines were stored safely during the two day inspection.

People were supported by sufficient staff who had gone through a thorough recruitment process. Staff had received appropriate training to enable them to fulfil their role and support the people living in the home. Systems were in place to ensure effective communication, including team meetings and staff one to one meetings with the senior management team. Staff spoke positively about the team and how they supported people. People told us they liked the staff and the other people they lived with.

The staff were knowledgeable about the people they supported and caring in their approach. Staff commented positively about the management support.

Systems were in place for monitoring the service. This included seeking the views of the people and their relatives through regular meetings and annual surveys.

During a routine inspection

We spoke with five people who used service, two staff on duty and one member of staff who contacted us after the inspection visit.

People said, 'Staff support me to go out when I want to', 'It's a good place to live, it never used to be, I used to get stressed but I don't anymore', 'Staff ask me about my care and I choose what I want to do', 'The resident meetings are really good, we get to discuss issues', 'It's alright here, I like it', 'Staff are helping me to get some work', 'I'm happy here', 'If I'm unhappy the staff come and sit and comfort me'.

Staff said, 'It's a good place to work', 'Since last September the place has been going in the right direction', 'My training needs are met, if I need to do anything else I will speak to the manager'.

A social worker said, 'There has been a massive improvement at the home', 'The home have worked really well with me and the Police', and 'Really impressed by the service'.

The interactions we saw between people and staff were friendly and respectful.

People's needs were assessed and person centred care plans and risk assessments gave an overview of how people should be supported.

The provider took appropriate steps to ensure that people were not at unnecessary risk of abuse.

Staff were employed in sufficient numbers and with the appropriate skills to meet people's needs.

Systems were in place that enabled people to give their opinions and enable the quality of the service to be monitored.

During a routine inspection

There were five people currently living in the home. We were able to discuss some aspects of the care provided with four people.

People told us they were being 'respected and involved in making decisions' and that their activities were discussed so that they were meaningful to them. One person told us that they were a member of the local allotment scheme. Care was personalised and risk assessed according to individualised needs.

People told us they 'felt safe' and they would report any signs of abuse to the staff.

People were supported to eat healthy and nutritional food and this was displayed on the menus. We heard a conversation between a staff member and one person about the importance of not missing meals and how it was important to eat breakfast.

People told us that staff were 'very supportive' and helped them with making 'complex decisions'. People were supported by a variety community health and social care professionals.

We spoke with a social worker who was at the home on the day of our visit. They told us, "I'm really impressed with the work that they are doing with the residents, they really do listen to them and treat them with respect'.

During an inspection in response to concerns

The people who use this service are young adults with mental health needs and have psychological input from a number of agencies.

Throughout the review people told us how they enjoyed living at the home. They told us 'we like living here very much it's the best place ever,' 'we can take turns to do the cooking there's a rota.'

People told us about the house rules, 'we have house rules which are to keep our rooms tidy and not to destroy the place.'

Several of the people were going out on activities and had planned to visit the cinema and the animal sanctuary where they were encouraged to walk and feed the dogs.
