During an assessment under our new approach

JS Care 4 All is a domiciliary care agency that provides personal care to people living in their own homes. At the time of the inspection, 31 people were receiving support with personal care. Not everyone who used the provider received personal care. The Care Quality Commission (CQC) only inspect where people receive personal care, this includes support with tasks related to personal hygiene and eating. This inspection took place on 19 February and 20 February 2026; when we visited the provider’s office. During the inspection we assessed all the quality statements under the key questions of safe, effective, caring, responsive and well led. Governance systems and oversight were not effectively established or operated, as they had failed to identify and address the shortfalls we found during this inspection. This placed people at potential risk of unsafe care and treatment. The provider could not be assured medicines were always managed safely, as appropriate systems to assess, monitor and mitigate risk were either not in place or not consistently implemented. Audits and monitoring were not always in place. PRN (as required) medication protocols were not in place at the time of inspection, increasing the risk of inappropriate or inconsistent administration. Recruitment systems were not robust as evidence explaining gaps in employment history had not consistently been obtained. Although this was addressed during the inspection, and the system was strengthened, this demonstrated a lack of oversight. There were no end-of-life care plans in place, meaning people’s preferences and needs at the end of life were not formally recorded. People had care and support plans in place; however, these were not person-centred and contained limited detail, which increased the risk that care would not be delivered in line with people’s needs and preferences. Consent forms were not always signed by the relevant person, meaning the provider could not be assured that care was always delivered with appropriate consent. This is the first inspection for this service since it was registered. This inspection identified that the provider was in breach of 1 legal regulation relating to good governance. After providing feedback to the provider, the registered manager sent us an initial action plan in response to the concerns we fed back. Safeguarding procedures were in place and staff knew how to recognise and report poor practice and abuse; they were confident in the manager to act immediately. Staff told us they felt people using the provider were safe. Staff were sufficiently skilled and experienced to deliver care. Staff spoke positively about their work and told us they felt supported by their registered manager and their colleagues. One staff member said, “I am proud to work as part of JS Care 4 All and remain committed to providing safe, respectful, and compassionate care.’’ We have asked the provider for an action plan in response to the concerns found at this inspection.
