Learning culture

Score: 3

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

Staff understood their responsibilities for ensuring safe systems of care including working with other professionals and services, to help ensure people received continuity of care. 

The registered manager explained how staff were confident in recognising and responding to any changes in people’s needs.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately.

Staff understood what safeguarding meant and how to take appropriate action. Staff had completed safeguarding training. A safeguarding policy was in place to provide staff with the most up to date guidance. One member of staff told us, “If I suspected any abuse, I would report it to the senior and manager right away.”

People told us they felt safe when receiving care from staff. One person told us, “Yes, I feel safe.” A relative confirmed, “[Name] is safe as the staff know what to do. I am reassured by that.”

Involving people to manage risks

The provider worked with people to understand and manage risks. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

Risks to people were balanced with their freedom and choices to ensure risk management was not unduly restrictive. The registered manager explained, “Risk management is an ongoing process, we regularly assess people’s risk.”

Staff were able to tells us how they managed risks to people. People’s care plans also contained guidance for staff on how to manage risks to people. A member of staff told us, “I have the same clients, I know them well and I know the risks they have and how to manage them.”

Safe environments

Score: 3

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

Processes were in place to help ensure the safety of people’s home environment and any equipment, such as mobility equipment. The registered manager explained that if any equipment (such as mobility equipment) was introduced, staff were provided with training and support to ensure they knew how to use it.

We spoke with an external professional who worked with the service, they told us, “If staff notice risks in the environment or with the equipment people are using, they let us know so we can go out and assess.”

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

Processes were in place to ensure staff were recruited safely and supported to perform their role in a safe and competent way. Staff had completed a full induction and all mandatory training.

The registered manager confirmed that staff underwent regular supervision processes when they started to help embed and develop their knowledge.

People told us they thought staff were well trained and they had good continuity of staff. One person told us, “Staff are on time, they always turn up and I have the same core staff team.”

 

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

Staff had been trained in safe infection control practices and had access to supplies of PPE. One member of staff told us, “I have had PPE and IPC training, there is plenty of PPE if we need it.”

 

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

People received their medicines in line with best practice. Staff who administered medicines to people had received appropriate training. Staff competency and spot checks were carried out to help ensure medicines were given to people safely and effectively.

One member of staff confirmed, “I have my competency checked, and I’ve had training, and this is refreshed once a year.”
