We looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.

At our last assessment, we rated this key question as inadequate. This rating remains in place and all scores remain the same. This inspection was to follow up on the enforcement warning notice and only considered those items stated in the notice. Therefore, only the quality statements ‘freedom to speak up’, ‘governance, management and sustainability’ and ‘learning, innovation and improvement’ were relevant to the content of the warning notice.

Freedom to speak up

Score: 1

Since the previous inspection, the freedom to speak up options had been promoted to staff to help improve their knowledge of the processes. Posters had been placed around the practice buildings and added to the practice online system to help staff know the options available. The process remained that staff contact an external guardian at the local medical committee (LMC). There remained no internal champions in place.

 

The practice had made some progress in improving their freedom to speak up processes, this would require monitoring in the coming months to ensure it worked effectively.

Workforce equality, diversity and inclusion

Governance, management and sustainability

Since the previous inspection, the practice had changed the way in which aspects of the practice management were recorded, monitored and overseen. This was reported to better enable managers to review and understand where the practice was up to with regards to complaints, incidents, appraisals, recruitment, induction, acting on risks, practice policies, meeting minutes, audits and general governance of the practice. During this inspection we reviewed this information and it appeared to be recorded more effectively, was accessible and user friendly.

 

The practice had made some progress in improving their governance, management and sustainability, this would require embedding and monitoring in the coming months to ensure it worked effectively.

Learning, improvement and innovation

Score: 1

Since the previous inspection, the practice had improved their systems of recognising, recording and monitoring incidents and complaints with the intention of ensuring learning would be derived from such events. Information about these was shared with staff through meeting minutes and there was a plan for these to be included in practice learning days.

 

The practice had caught up on outstanding appraisals and had introduced a documented system of induction for new staff. The supervision of Advanced Nurse Practitioners was a work in progress and formal documentation and a review of consultations, prescribing and record keeping was planned and a policy was being drawn up.

 

The practice had made some progress towards improving their learning, improvement innovation, this would require some more progress and then monitoring in the coming months to ensure it worked effectively.
