18 and 25 February 2015

During a routine inspection

Sydenham House Residential Home is a care home registered to accommodate up to 19 older people. At the time of our inspection 18 people were using the service.

This inspection was unannounced and took place on 18 and 25 February 2015.

There was a registered manager in post. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are registered persons. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

People were safe because the registered manager and staff team understood their role and responsibilities to keep people safe from harm. Staff knew how to raise any concerns regarding people’s safety. People were supported to take appropriate risks and promote their independence. Risks were assessed and individual plans put in plans to protect people from harm. People were protected from the risks associated with medicine because the provider had clear systems in place and staff had received the appropriate training. Employment checks were carried out on staff before they started work to assess their suitability.

People were provided with effective care and support. Staff had received the appropriate training to meet people’s needs. People were supported to eat and drink to maintain an appropriate body weight and remain hydrated. Arrangements were made for people to see their GP and other healthcare professionals when they needed to do so.

People received a service that was caring. They were looked after by care staff that were familiar with their needs and wishes. People were involved in making decisions about how they wanted to be looked after and how they spent their time. People had positive relationships with the staff caring for them. Staff treated people with dignity and respect.

People received person centred care and support. They were offered a range of activities both at the service and in the local community. People were encouraged to make their views known and the service responded by making changes.

The service was well led. The registered manager provided good leadership and management. The vision and culture of the service was clearly communicated. The quality of service people received was monitored on a regular basis and where shortfalls were identified they were acted upon.

During a routine inspection

People told us that staff were respectful and asked their permission before providing assistance with their personal care. It was evident that people maintained autonomy in their day to day lives as far as they were able to do so.

We saw that people's weight was monitored regularly and risk assessments had been carried out regarding people's nutritional status. A staff member said, 'People seem to like the food at the home' and told us that 'meals always look appetising'.

The home had experienced particular problems with the supply of medications from one GP practice and were addressing this at the time of the inspection. Despite this, staff had identified any discrepancies and had acted in a timely manner to ensure people's medicines were available and given as prescribed.

We found that staff had completed training relevant to their roles and to meet the needs of people living at Sydenham House. People told us that they were happy with the support they received from staff and that staff knew how to help them.

None of the five people we spoke with had needed to raise a complaint. One person said, 'I am aware that there are problems in some homes' there are no such concerns here'.

In this report the name of a registered manager appears who was not in post and not managing the regulated activities at this location at the time of the inspection. Their name appears because they were still a Registered Manager on our register at the time.

During a routine inspection

In this report the name of the registered manager appears who was not in post and not managing the regulatory activities at this location at the time of the inspection. Their name appears because they were still a Registered Manager on our register at the time of this inspection. We have advised the provider of what they need to do to remove the individual's name from our register.

We looked at the care records for five people who used the service. The care plans were in place but had not been reviewed. We saw evidence that one person's nutritional assessment had been recorded monthly. We also saw that activity records were present and completed. Staff were observed supporting people in daily activities and routines.

During our visited we noted people who used the service were clean and well presented. We spoke to three of these people and they all told us how well they were looked after by the staff. One person said 'it's ideal; the staff always help me if I need it. It's not like your own home but it's as near as you can get'. Another person told us that they had plenty to do each day.

During a routine inspection

People who live in the home told us that staff were "all very friendly and helpful", "I feel quite at home here", "everything is just right for me, get what we want".

One individual told us that "if I was unhappy about anything I would tell one of the staff and they would try and do what they can". Staff "all very good, treat us with respect".

We were told "I feel like this is my home" "very happy here" "I like it because you can choose what you do theres no routines".

We were told by a visitor that its "a welcoming home with friendly staff".

We were provided with some comments received from relatives and carers: "thank you for looking after our relative so well", "totally re-assured she was happy and well looked after and certainly healthier", "all your staff are dedicated and special".
