Condition of this registration relating to carrying out this regulated activity
a. The registered provider must maintain oversight of the practice management at The Sidings Medical Practice and ensure there is sufficient managerial capacity within the practice to ensure effective governance is carried out. The registered provider must demonstrate how their governance system will monitor that actions taken to address the issues outlined in this proposal are sustained.
b. The Registered Provider must have a fully implemented plan to provide monitoring and care to patients living with a long-term condition within 28 days.
c. The registered provider must implement a plan within 28 days to provide all patients prescribed ongoing medication including high risk medicine with appropriate monitoring and review by an appropriately qualified clinician.
d. The registered provider must implement a system that allows blood results to be reviewed and appropriately managed by a suitably qualified clinician within 28 days. This must include a review of abnormal results to check for historic missed diagnosis.

e. The total number of ongoing correspondence received by the practice on a daily basis averaged over a week.
The registered provider must confirm to the Care Quality Commission by the 15 December 2023 that a suitability qualified, competent, skilled, and experienced person is available 5 days a week to provide leadership and oversight of The Sidings Medical Practice and the conditions imposed within this notice.
a. The registered provider must maintain oversight of the practice management at The Sidings Medical Practice and ensure there is sufficient managerial capacity within the practice to ensure effective governance is carried out.
