During an inspection looking at part of the service

We carried out this focused inspection under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. This inspection was planned to check whether the provider is meeting the legal requirements and regulations associated with the Health and Social Care Act 2008, to look at the overall quality of the service, and to provide a rating for the service under the Care Act 2014.

We carried out an unannounced focused inspection of this service on 21 December 2015. A breach of legal requirements was found following the comprehensive inspection on 5 October 2015, the provider wrote to us to say what they would do to meet legal requirements in relation to the breach.

We undertook this focused inspection to check that they had followed their plan and to confirm that they now meet legal requirements. This report only covers our findings in relation to this requirement. At the last inspection on 5 October 2015 we asked the provider to take action to make improvements. We asked the provider to provide staff with a minimum of six one to one supervision sessions annually. During this inspection we found the provider to be compliant.

The service had a registered manager in place. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

6 & 8 October 2015

During a routine inspection

This inspection took place on 6 and 8 October 2015 and was unannounced. This meant the provider was did not know we were coming.

We last inspected Pelton Grange on 4 December 2013 and found it was compliant with the regulations in force at the time.

Pelton Grange is registered with the Care Quality Commission (CQC) to provide care for up to 43 elderly people. The home also provides nursing care. At the time of our inspection there were 37 people living in the home. The provider had recently altered two single bedrooms on the first floor to create an office and a kitchen. Another bedroom on the ground floor was also used as an office. Some other bedrooms that in the past had been used as double rooms were now redesigned for single occupancy. This meant that the number of rooms available for occupancy was now 38.

Five beds were contracted by the local CCG and are designated as ‘Intermediate Care’ beds to facilitate early discharge and reduce admissions to hospital. Care is provided by staff employed by the home with intensive support by intermediate care staff who visit regularly. These include physiotherapists, occupational therapists and specialist nurses.

During our inspection we found the previous registered manager had retired and left the service and a new acting manager had been appointed. On the day of our inspection the acting manager had been in post for four weeks. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

We found staffing levels at the home were appropriate for the number of people living there.

We found people’s medicines were well managed.

We saw the home had in place personal emergency evacuation plans displayed close to the main entrance and accessible to emergency rescue services.

We found the home had cleaning schedules in place to prevent the spread of infection.

The registered provider had worked within the Mental Capacity Act 2005. We saw that all people living at Pelton Grange had been asked for consent to support and Mental Capacity Act assessments to identify their capacity to consent to their care had been carried out. We also saw Deprivation of Liberty Safeguards were in place where required.

We observed staff speaking with people in kind, respectful and reassuring ways. People told us they felt their dignity and privacy were respected by staff.

We saw a notice board on which was displayed information about the activities for that week. During our inspection we found various activities were taking place.

We found the provider had audits in place to measure and monitor the quality of the service. The provider had in place a complaints policy in place and this was clearly displayed for people to see. We found that complaints were investigated appropriately.

During our inspection we found staff had not received regular one to one supervision sessions with the previous registered manager who had retired four weeks before our inspection visit.

This is a breach of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010 and under the Care Act 2014 Regulation 18 (1) (a). You can see at the back of this report what actions we have asked the provider to take.

4 December 2013

During an inspection looking at part of the service

At our last inspection carried out on 9th April 2013 we identified concerns with outcome 8 (HSCA Regulation 12) cleanliness and infection control. We found the provider was not providing effective, safe and appropriate systems to detect and control the spread of health care associated infections.

During this inspection we found the provider had made significant improvements to ensure people were protected from the risk of infection because appropriate guidance had been implemented and followed.

We saw all areas including laundry, kitchen, lounges and bedrooms were clean, pleasant and odour-free. Staff confirmed they had received training in infection control. The records we looked at confirmed this.

Five people who used the service told us, the home is always nice and clean. One said, 'My bedroom is always tidy with fresh clean sheets on their bed.' Another said, "All the bathrooms and toilets are always nice and clean and fresh. I think the domestic staff do an excellent job."

9 April 2013

During a routine inspection

We spent time observing how staff supported people who lived at the home. People told us they were happy with the care provided. Comments included 'I'm extremely impressed with the care; this is a beautiful place and I am very content". It's one of the best I've seen (relative's name) for a long time", 'The staff, they're very good' and 'It's so nice here, they look after me extremely well, I have no complaints at all.'

A professional visiting the home said "The care here is first class, I would recommend this home to anyone."

We looked at staffing levels within the home. One person said 'There seems to be enough, but we could do with some more because they all work really hard and are always very busy', another said 'Yes, I never have to wait too long for anything.'

We watched how the staff supported the people in their care. We saw people were treated with dignity and respect. For example, when staff talked with people they made sure that they got down to their eye level. We heard staff address people respectfully, speaking quietly about private matters.

People said that they knew they could speak to a member of staff if they had a complaint. One person said "I would speak to the manager if I wasn't happy about something", another said 'If we have any concerns at all we can tell them.'

8 November 2012

During a routine inspection

People told us they were happy with the care and support they received at Pelton Grange Care Home. We found that people's care and support needs were appropriately assessed and their care was planned. One person said, "They look after me well here." Another person said "I think the care here is wonderful. The staff are exceptional, nothing is too much bother for them, they are cheerful and fun." A third person told us, "I couldn't complain about anything, they look after me very well."

People told us their consent was gained prior to care being delivered and we found care was delivered safely and to an appropriate standard. There were sufficient numbers of suitably qualified, skilled and experienced staff on duty to meet people's needs and safeguard their health, safety and welfare. We saw the provider had a complaints policy and procedure in place.

However, although people who received care and support from Pelton Grange Care Home told us they were happy, and we saw they were well supported, we found that failures to maintain the premises suitably, may put people's safety at risk.

During a routine inspection

We visited this location on a weekday and were able to talk to four people who used the service. We were told that the staff were 'really good' and that people were 'spoken to well', that staff were 'respectful' and that the food was 'lovely' and that there was 'plenty'. People who used the service also said that they did not have concerns and they confirmed that they knew how to discuss issues and with whom.
