Person-centred Care

Staff provided person-centred care which met people’s needs and reflected their preferences. People told us they were happy with the support they received. A person commented, “I think it is amazing care.” Staff created care plans for each person. These focussed on their individual needs. Care plans were reviewed monthly or more often if needed. People were asked for their views about their care and were able to request changes.

External professionals told us staff were responsive to people’s changing needs. A visiting professional commented, “Staff are very good. They know how to support people with dementia. They are always interacting with people. I believe people get a better quality of care than in other places I visit.”

Care provision, Integration and continuity

Staff supported people to have joined up care with other services. They worked closely with other professionals and shared information with them. Staff supported people to attend appointments when needed.

Providing Information

People had the information they needed about the service. They were involved in planning their care and had information about this. Staff shared verbal, pictorial and written information about activities, events, menus and other aspects of day-to-day life at the home. The chef and activities coordinator regularly spoke with people about different choices and involved them in making decisions.

Listening to and involving people

The provider made sure they listened to and involved people in decisions about their care and the service. There were regular meetings for people who lived at the home. They told us they were able to speak with the registered manager when they wanted. They felt listened to and valued.

Equity in access

Staff supported people to access other services when they needed. For example, escorting them on appointments and organising transport when they needed this.

Equity in experiences and outcomes

Staff completed training about equality and diversity. There were appropriate procedures in place. People told us they did not experience discrimination at the service.

Planning for the future

Score: 3

People were supported to plan for the future. They were asked about wishes for end-of-life care and in the event of their death. Staff helped people plan to meet these wishes. Nurses were trained to give specialist medicines for people who needed this to provide pain relief and comfort at the end of their lives. Staff worked closely with palliative care teams and other professionals.
