During an inspection in response to concerns

We completed our inspection on the inpatient ward only.

We ask five key questions of services we inspect. Are they safe, are they effective, are they caring, are they responsive and are they well-led? Because this was a responsive inspection we focused on two of those five questions; is the service effective and are they well led?

Is the service effective?

We found the service was not effective in relation to monitoring and assessing pressure area care. Do not attempt cardiopulmonary resuscitation orders (DNACPR) were also not appropriately completed and care plans lacked person centred information to ensure effective care could be provided.

Is the service well led?

Governance and quality assurance processes were in place at this hospital. We found that action was taken in relation to patient's feedback, regular auditing took place and the service monitored and analysed its incidents. However, the provider may find it useful to note that there was limited evidence provided to us which demonstrated that learning and improvement took place from incident trends or serious incidents.
