During an inspection in response to concerns

People who used the service, who were able to, told us that they were not happy with their care and treatment. They did not think they were treated with dignity or that their choices and wishes were respected. One person told us "Since the manager left, this is not a nice place to live, the carers do their best but they are always busy and everything is rushed. Sometimes the nurses are not good at giving me the care I need".

We found that records were not accessible and there was a lack of information in one care plan to show how people were being cared for properly and safely. Records we did see were not up to date and did not include all the necessary documentation.

We saw that most people preferred to stay in their rooms all day and there were no activities available. People told us and we saw that people did not have the opportunity to interact with others.

During an inspection looking at part of the service

This was a follow up inspection visit to check that concerns noted previously had been addressed.

During the present inspection people who use services said that the staff treated them with respect, involved them in decision making and provided them with the nursing and personal care they needed. One person said, 'The staff here are excellent and nothing at all is too much trouble for them. A carer (relative) said, 'The staff are wonderful. It's not just a job for them, they really do care and they treat the residents with genuine affection'.

During an inspection looking at part of the service

People using the service told us that they felt safe in the home and that the atmosphere had improved since one member of staff had left. However, they were unclear whether the staff member would be returning to the home and expressed anxiety about this. They said that generally the care they received was good, but that there were not enough staff available to meet their needs. People gave us several examples of where they had to wait for staff when they needed help.

During a routine inspection

People using the service told us that they had been able to make their own decision to move to the service and had been able to trial it for a month before deciding to stay. They had been involved in the assessment of their needs and how these would be met. People told us they felt safe in the home and were satisfied that they were cared for in a way that respected their privacy and dignity. They also told us that they could not fault the nursing care provided.

People told us that the staff were 'excellent'. They also told us that the activity worker was 'fantastic' and that 'she comes around and asks us what sort of activities and entertainment we would like'. However, people also told us that there was a high turnover of staff and that staff morale was low. They told us that there was often shouting between the senior nurse and the manager and that staff were shouted at by the senior nurse, which made the atmosphere in the home unpleasant at times.

People using the service were happy with the cleanliness of the home and the facilities available to them, although they told us that the home was sometimes cold. They commented that there was not enough staff on duty to do the laundry and the supper and that this made the staff 'rushed off their feet'. They said they enjoyed their meals and had plenty of choice for lunch, but that they would have liked a wider choice for supper.

People knew how to make a complaint if they needed to and told us that the manager regularly spoke with them to ask how things were going.
