Patients, and where appropriate those close to them, were involved in assessments of their needs. Staff reviewed assessments taking account of the patient’s physical emotional, communication and personal care needs. Care and treatment were based on latest evidence and good practice guidance. We reviewed policies and patient records which supported this. Patient outcomes met expected standards, with audit data indicating low levels of harm. Patients’ pain was quickly identified and well managed.
Staff assessed the patient’s nutritional and hydration needs and made sure patients were able to eat and drink sufficiently. Staff made sure patients understood their care and treatment to enable them to give informed consent.
