Well-led
Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.
At our last assessment we rated this key question requires improvement. This remains the same as it was a targeted inspection. This meant the management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care.
The service was previously in breach of the legal regulation in relation to good governance.
Improvement was found at this assessment, and the provider was no longer in breach of this regulation.

Governance, management and sustainability
The provider had not always had clear responsibilities, roles, systems of accountability or good governance. They had not always acted on the best information about risk, performance and outcomes, or shared this securely with others when appropriate. However, systems and processes were now in place or in the process of being implemented which would continue to support the service on their journey of improvement. We did identify some issues during our site visit, but these had been picked up by the providers quality assurance systems. We found the new robust quality assurance systems were being embedded into practice by the new management team, the systems were identifying issues and action plans had been developed to ensure improvements were made. The manager and clinical lead were passionate about ensuring these were sustained to ensure people received the best possible care and support.
