During an assessment under our new approach

Date of Assessment: 24 July to 14 August 2025. We visited the service on 24 July 2025.

Woodhill Grange is a care home providing personal and nursing care to older people, some of whom were living with dementia. At the time of this assessment there were 59 people living at Woodhill Grange.

This assessment was targeted to follow up on actions required after the last assessment. We looked at medicines management and governance. The previous rating was requires improvement, this was a targeted inspection therefore, the rating will not change.

At the last assessment the provider was found to be in breach of the legal regulations relating to medicines management, person centred care and good governance. We looked at medicines management and governance as part of this targeted assessment and found improvements, and the provider is no longer in breach of regulations in these areas.

We found improvements had been made within the service. A new manager had been recruited, and the acting manager was appointed as the clinical lead. The provider had also recruited a deputy manager, which ensured a strong leadership team. The provider had worked hard to make improvements. However, some systems and processes needed time to be fully embedded and sustained to continue to drive improvement.

Care systems and processes developed provided clearer oversight of the service. Care plans, care records and risk assessments were more detailed, and person centred. Staff told us they now felt supported and were receiving regular support and supervision from the new manager.

The service made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. They involved people in planning, including when changes happened.

Date of Assessment: 15 January 2025 to 30 January 2025. Wood Hill Grange is a care home providing accommodation for nursing and personal care to older people, some of whom are living with dementia. The service can support up to 75 people. At the time of this inspection there were 60 people living at Wood Hill Grange. At the last inspection we looked at safe and well-led and did not provide an overall rating. The service was rated requires improvement in safe and was in breach of regulation 12, medicines management. At this inspection we found improvements were still required in the management of medicines and identified additional breaches in regulations 9 and 17. We found concerns regarding person-centred care, risk management and governance. The service is rated requires improvement overall.

During an assessment under our new approach

Wood Hill Grange is a care home providing accommodation for nursing and personal care to older people, some of whom are living with dementia. The service can support up to 75 people over 4 units, each with a separate dining room and lounge. At the time of this inspection there were 42 people living at Wood Hill Grange and one unit was closed. This is the first rated service for the new provider. We rated this service under the previous provider and under our previous methodology in January 2020, where it was inspected and rated good. We carried out this assessment in March 2024. We looked at all the quality statements under the key questions, Safe and Well-led. We also looked at the following quality statements, assessing needs, Independence, choice and control, person centred care and Equity in experiences and outcomes. We found improvement was required in some areas. We found the service rated requires improvement in Safe, however, was good overall.
