During a routine inspection

During our inspection to 3 Barn Rise, we interviewed three support workers. We spoke to the registered manager and observed care and support provided by staff. We viewed two care plans and other relevant documents.

The care we observed was compassionate and ensured people's safety and welfare was met.

We found that people were well cared for and their needs were met. The provider acted in accordance with legal requirements to seek consent for care and treatment and people were offered choices whenever possible.

Care plans were up to date and people who used the service and significant others were involved in the review process.

Meals provided were well balanced, nutritious and culturally appropriate. People who used the service were involved in the planning of the menu and shopping of the ingredients.

We observed people taking part in community based activities, for example during our inspection people went shopping for Christmas presents.

We noted that the building had structural problems which were in the process of being addressed, however the environment looked very tired. A survey undertaken in February 2013 stated that work was required to redecorate and refurbish the home, but we found no action had been taken.

A longstanding staff team ensured consistency in the provision of care.

People who used the service had no complaints about the treatment and care provided. Staff were confident that the Registered Manager would respond appropriately to all complaints raised.

During a routine inspection

On the day of our visit most of the people who use the service were out engaging in activities in the community. The people who were at home had complex needs which meant they were not able to tell us their experiences.

The provider had appropriate systems in place to ensure medications were managed safely. We also found that staff had sufficient support and training to ensure they were able to perform their role. The service had robust systems in place to ensure the correct number of staff worked each shift to meet the needs of people who used the service.

We found care plans and risk assessments reflected individual needs of people and were frequently reviewed to ensure they were current. There were policies and procedures for staff to follow in order to minimise the risk of inappropriate treatment. Complaints were appropriately recorded and investigated.

During a routine inspection

People living in 3 Barn Rise are non-verbal or pre-verbal using one or two words or short phrases. It was therefore difficult to obtain verbal account from people on their experience of living in 3 Barn Rise. We based our judgement on observations we made during our visits. We observed people in their home and took part in the daily run to Willesden Resource Centre (WRC) a day service managed by Milbury Care Services.

We observed staff treating people using the service with respect and involving them in the care. Staff asked people if they wanted to go to the day centre and where they wanted to sit on the bus. Staff were very patient with people and gave all people appropriate time to make up their mind and make their decisions.

During lunch time people were seen to make independent choices of what they would want to eat and were able to refuse meals offered by staff.

We observed staff ensuring that people were safe when using the car and all seatbelts were fastened safely.

People using the service were comfortable with staff and appeared relaxed in their company.

To ensure that care is provided to people consistently we observed staff handing over important information to staff in WRC.

We observed staff asking people using the service if they were ready to take their medication enabling them to make a choice and take greater part in the administration of medicines.
