During an inspection looking at part of the service
We found that there were enough qualified, skilled and experienced staff to meet people's needs. People were cared for by staff who were supported to deliver care and treatment safely and to an appropriate standard.
We found that the provider did not have an effective system to regularly assess and monitor the quality of service that people receive. We found that the provider had addressed the majority of concerns we had highlighted during our previous inspection. However, further improvements were still needed.
We found that policies were not always up to date and reflective of current staff practice. We found that incidents were reviewed and remedial actions were put in place to minimise the immediate risk to patients but the process for investigating incidents to prevent reoccurrence was not effective.
We found that the provider assessed and monitored key risks; however, the processes for monitoring clinical governance were not fully in place. We found that where actions plans were put in place, these did not always identify the person(s) responsible for completion of the actions and the timelines for completion of actions were not documented.
