During a routine inspection

The Mews is a purpose built care home which provides nursing and personal care for older people, some of whom may be living with dementia. It is registered to provide up to 50 places, but two rooms are currently used as an office and a day room. All of the bedrooms are for single occupancy and are en-suite. At the time of this visit there were 41 people living at the home, including two people on short break placements.

The last inspection of this home was carried out on 20 August 2013. The service met the regulations we inspected against at that time.

This inspection took place over two days. The first visit on 24 February 2015 was unannounced which meant the provider and staff did not know we were coming. Another visit was made on 25 February 2015.

The home had a registered manager who had been in this role for two years. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

People told us they felt safe at the home and were happy with the staff. One person commented, "Yes, I am safe - everything is fine." Relatives also felt their family members were safe and comfortable with the staff who supported them. The health and social care professionals we spoke with during the inspection told us they had no concerns about the safety of people using the service. One visiting nurse told us, “I come at all different times of the day and have never had any concerns.”

Staff were clear about how to recognise and report any suspicions of abuse. Staff told us they were confident that any concerns would be listened to and investigated to make sure people were protected. The provider made sure only suitable staff were employed. People were assisted with their medicines in the right way.

All the people we spoke with felt their care needs were being met. One person felt they had to wait for assistance because staff had to bring hoist equipment. We told the registered manager to make sure staff and equipment were deployed as quickly as possible when people requested support. Staff told us there were enough staff on duty to meet people’s physical and social needs. Visiting care professionals also felt there were enough staff to support people in the right way.

The people we spoke with confirmed that staff met their care needs. People’s comments included, “They are very good and will give extra help if necessary” and “they couldn’t do any more than what they do”. All the people and relatives we spoke with felt staff had the right skills and competencies to provide the right support. Staff had the relevant training and support to care for people in the right way. Staff understood the Mental Capacity Act 2005 for people who lacked capacity to make a decision.

People said any changes in their health needs were referred to the relevant health care services. Health care professionals said the home responded appropriately to any changes in people’s well-being.

People were supported to eat and drink enough and they had choices about their meals. People and relatives felt staff were caring and kind. People were encouraged to make their own decisions and choices about their lifestyle and daily routines. There was a warm sociable atmosphere in the home and there were friendly interactions between people and staff. People had opportunities to join in activities or go out with staff from time to time.

People had information about how to make a complaint or comment and said they would be confident that these would be acted upon. People, relatives and staff felt they could approach the registered manager at any time and said the home was “well managed”.

Staff felt there was an open and supportive culture amongst the staff team. They said they liked working at the home. The provider had an effective system for checking the quality and safety of the service.

During a routine inspection

There are two managers recorded as registered for this service. We are currently removing Mr Proud as he has informed us he is no longer carrying out the role.

We spoke with fourteen people, they told us they were happy with their care. One person said 'I can get up when I want and am under no pressure to do things' and 'the staff are helpful and respectful'. A relative said 'The staff are good, nice with my husband and listen to what you say'. Due to peoples complex needs we used different methods to help us understand their experiences including observing people's care.

People spoken with were very positive about the care they received and complementary about the staff. Relatives said they were involved in developing the care plan and were happy with them. Neither had wanted to see the records on a regular basis.

Staff received training in safeguarding vulnerable adults and were confident that they could identify any issues and would act upon any concerns. The manager was aware of her responsibilities in these circumstances. There were sufficient staff on duty on the day to meet people's needs and the staff rota showed these numbers were maintained.

There were systems for auditing and monitoring the service. For example audits, meetings with people using the service and their families and with staff. Surveys were also carried out. The records of these processes were up to date and gave the manager and company information they needed to run the home effectively.

During an inspection looking at part of the service

We undertook a visit as part of a compliance review for The Mews Care Home on 14 August 2012. This was to review the improvements made following the inspection we carried out in April 2012, when we found that some improvements were needed. At that review we found that although most of people's views and experiences were taken into account in the way the service was delivered they could not always be in involved in making decisions about the choice and variety of the food they were being offered. Although there were effective systems to regularly assess and monitor the quality of service that people received, some issues had not been responded to at that time.

We spent some time talking to the people living in The Mews, their relatives and the staff and we observed the day to day care being delivered as well as the lunch time meal. Some of the people had physical and mental health needs meaning that it was not possible to get some people's views.

All of the people living in the home who we spoke with said they were happy with the service provided by the staff. A person living in the service told us that the staff were 'really good.'

We spoke to a visitor of a person who had been in the home for a short time, they told us that they were 'More than happy' with the care their relative had received and that they 'Could not speak highly enough of the home and the staff working in it. They singled out particular staff who had been particularly helpful and 'Kind' and said that they 'Could not have done more'.

One visiting relative told us they were confident that their family member was being looked after. When we asked about the care being given to their relative, they said, 'Its great'.

During a routine inspection

We, undertook a visit as part of a compliance review for The Mews Care Home on 26 and 27 April 2012.

Due to the physical and mental health needs of the people living in the home it was not possible to get some people's views. However we were able to speak to a number of the people living there and we spoke to visiting relatives, representatives and visiting professionals.

All of the people living in the home who we spoken with said they were happy with the service provided by the staff. A person living in the service told us that the staff were 'really good.' A visitor said, 'We are sure that my (relative) is getting good care.'

One visiting relative told us they were confident that their family member was being looked after. When we asked about the care being given to their relative, another person said, 'It has got better in the recent months.' One person was not clear how much social interaction their relative was supported to have and we directed them to the PAL (social activities co-ordiator) for them to discuss this further.

During a routine inspection

All of the people living in the home who were spoken to said that they were very happy with the service provided by the staff. They said that staff responded promptly and politely to any requests for assistance. One person said that she felt that the staff 'always ask me what I want to do' and that she was very satisfied with the 'way she was looked after' another said that the staff treated her with 'kindness and was always nice to her'. People said that they were asked about what help they needed when they started to receive the service and consulted about any changes in their care provision. All of those spoken to were aware that they had a care plan but none had wanted to see it and were happy with the way that the staff discussed the contents with them when any changes were made. The relatives spoken to said they had been consulted about the care plans. One relative said that 'the Manager is very good and has a very public way of keeping people involved'; another said that she always found the staff 'very helpful and caring'.

People said that they thought that the staff knew their jobs and had the training they needed. One said 'the staff are always talking about this training and that training'. They said that the food was good and that there was always a choice at mealtimes, one said 'I have put weight on since I came in, but I'm not complaining'.
