Our current view of the service

Inadequate

Date of the assessment 3 March 2026 to 25 March 2026. Evergreen Lodge is a care home with nursing, which providers accommodation and personal care as a single package under one contractual agreement. The service is a 40 bedded care home split over 4 units and 2 floors. At the time of the assessment the service was at maximum capacity.
During the assessment we looked at all the 5 key questions.
The provider was in breach of regulations relating to dignity and respect, consent to care, safe care and treatment, good governance and staffing.
The provider did not adhere to the principles of the Mental Capacity Act. There was no evidence that all practical steps had been taken to support people in making their own decisions. There was no evidence that people had been provided with information in a way they could understand to support them to make decisions.
The best interest process had not always been followed to ensure decisions were made in the person’s best interest and that appropriate people were involved in the decision-making process.
When a person required the use of restraint to protect their well-being and safety, this was not always clearly recorded and there was little evidence a debrief did had taken place. Debriefs are important for peoples emotional and psychological wellbeing as well as learning and reflection.
Accident and incidents were not always fully recorded and lacked lessons learned to drive improvement and mitigate further risks.
We observed positive interactions between staff and people who lived in the home. Permanent staff knew people well and this was evident through their interactions.

People's experience of the service

Relatives we spoke with were happy with the support being provided to their loved ones. Some people were not able to directly tell us about their experience therefore we completed observations to help us to understand people experiences of care. This approach demonstrated people were not always treated as individuals, support was provided in a way that benefitted the service rather than meeting individual needs. Routines and restrictions were in place that sometimes these had a negative impact on the people living within the service. We identified some people had to wait for care due to staff not being available to provide support. This included support in relation to personal care.
People told us they had to wait for things that mattered to them, such as their cigarettes and were not allowed to smoke at night. We observed people waiting and being told to wait until their scheduled cigarette time.
We observed people being weighed in communal areas and their personal information being discussed. This compromised their dignity and confidentiality.
There were mixed observations regarding the support provided by staff. Some staff explained the support they were providing and gained consent from people, however at times staff were observed providing support without engaging with the person or explaining the care being given.
We also observed positive interactions between staff and people living and the service.
