Monitoring and improving outcomes

The provider did not always routinely monitor people’s care and support to continuously improve it. People’s care plans did not always clearly demonstrate how they were being supported to plan, monitor and achieve meaningful goals and outcomes. The provider had introduced keyworker meetings to support people’s involvement in reviewing their care and discussing their goals. However, records showed these meetings were not always taking place regularly.

Following our feedback, the provider acted promptly and confirmed they were introducing additional checks to ensure keyworker meetings were completed as scheduled and people’s goals were more clearly reflected in their care plans.
 

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.
