Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People’s needs were assessed before care was provided and their care and support was reviewed regularly. Assessments were holistic and reflected people’s physical, mental health and social needs. Care plans were based on these assessments and provided clear guidance for staff to deliver effective care. Care plans reflected people’s preferences, such as daily routines, favourite activities, and how they wished to be addressed. One person’s relative told us, “[Person]’s care plan takes into account their preferences, their likes and dislikes, and their ethics”.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.

People received care and support based on current guidance and good practice. Recognised tools were used to assess risks such as skin integrity, falls, eating and drinking, and health conditions. Where risks were identified, care plans were in place to manage and mitigate risks to people. Staff followed clear procedures and guidance to ensure care was delivered safely and effectively.
 

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.

People’s care plans and individual risk assessments were accessible to care staff. These contained all the information needed to deliver people’s care and support them appropriately. Staff worked well with each other and external professionals to meet people’s needs. Information was shared appropriately to support joined-up care. The provider worked in partnership with people, their families, and relevant health and social care professionals. This helped ensure people experienced consistent and effective support.
 

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

People were supported to maintain good health and wellbeing. Staff recognised changes in people’s health and took appropriate action, including seeking advice from healthcare professionals where needed. Staff escalated concerns to the management team appropriately and supported people to access services to support their health and wellbeing. This helped to promote positive outcomes.
 

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

People’s care and support was regularly reviewed to ensure their needs were met, and changes in health and wellbeing were identified and responded to appropriately. Staff understood their roles in monitoring people’s wellbeing and took action where concerns were identified. The provider regularly reviewed people’s care plans, risk assessments and daily notes to identify any patterns or changes which might indicate a developing need. This approach helped ensure people experienced positive outcomes and received care which remained effective over time.
 

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

Staff understood the importance of gaining consent when providing people’s care and respected people’s decisions. Where required, the provider followed relevant legislation and guidance. People were supported to make their own choices, and decisions were recorded appropriately. One person’s relative told us, “Staff always ask what we want to do in relation to [person]’s care. They do not take these things for granted”.
