Accommodation for persons who require nursing or personal care

Condition of this registration relating to carrying out this regulated activity
The registered provider must not admit any new service users to Swan House, Pooles Lane, Short Heath, Willenhall, WV12 5HJ without the prior written agreement of the Care Quality Commission. This includes any respite or emergency admissions and re-admissions of any service user who has previously been resident at Swan House.
The registered provider must ensure that all service users receiving nursing care have their individual needs reviewed and risk assessed within three working days of the date of this notice. These risks are not limited to but must include; falls, choking, behaviours associated with living with dementia and malnutrition/weight loss. Evidence of accurate and up-to-date care plans, risk assessments and completed medicines administration records must be submitted to the Commission within five working days of the date of this notice, for all service users.
The registered provider within five working days from the date of this notice must ensure effective systems are in place to manage service users who are mobile and living with dementia, utilising specialist advice from a multidisciplinary professional team to assess and mitigate the risks relating to behaviours that are associated with people living with dementia. These behaviours include; unsupervised service users entering the bedrooms of other service users when the service user is present and physical and verbal aggression.
The registered provider must ensure that all staff have received training specific to their roles [including training in Positive Behavioural Support (including sexualised behaviours), advanced and complex dementia and mental health] and competency checks for the role which they are to undertake. Evidence of this training and checks must be submitted to the Commission within five working days of the date of this notice together with an action plan setting out any further training required and the timescales for this.
The registered provider must ensure effective quality assurance processes are established and implemented to improve the way you assess, monitor and improve the quality of the service and protect service users from the risk of harm. This is not limited to but must include; systems to check equipment used in risk mitigation plans is in good working order, systems to check staff are following risk mitigation plans and a system to ensure risk assessments and mitigation plans are accurate, up to date and reviewed following incidents. Evidence of your quality assurance framework must be submitted to the Commission within five working days of the date of this notice. Thereafter, the registered provider must send a report to the Commission on the first day of each month with details of completed quality monitoring audits and checks. This must include areas covered, your findings and any responsive actions, clearly identifying who is responsible and the timescales for this.
The registered person must not provide nursing care under Accommodation for persons who require nursing or personal care or Treatment of disease, disorder or injury.
