Our current view of the service

Date of assessment: 28 January to 2 February 2026. Gracefields Nursing Home is a care home providing care and support to older adults, some of whom were living with dementia. The service provides care and nursing support for up to 50 people. At the time of the assessment, there were 45 people using the service. At our last assessment, published in March 2020, the service was rated as good. The service had a manager but they were not yet registered with CQC. The service had clear systems in place for managing concerns and complaints, and the management team worked to streamline processes where duplication made oversight more difficult. Feedback about the manager was consistently positive, particularly regarding their open‑door approach and responsiveness to issues raised. Staff spoke positively about working in the service and were observed to treat people with dignity and respect. The environment was clean, tidy and well maintained. Although improvements were identified in relation to medicines, governance and staff supervision, there was no evidence that these shortfalls had resulted in harm.

People's experience of the service

We spoke with 12 people who used the service and 7 relatives about their experience of the care provided. Some people were unable to share their experiences directly, so we spent time observing interactions with staff. To support this, we used the Short Observational Framework for Inspection (SOFI), this is an observation tool which helps us understand people’s experiences and assess whether they receive good care. Relatives shared mixed feedback about the service, though many told us they were generally happy with the care provided. During our visit we observed a calm and supportive lunchtime, with staff offering gentle reassurance, clear communication and timely assistance. Most people appeared settled, some smiling or engaging with the music, and those who declined food were offered suitable alternatives. People and relatives spoke positively about the cleanliness of the home, while views on food and activities varied. There was some awareness of resident meetings, and records showed these were taking place and informing improvements.
