Our rating of this key question went up we rated Effective as Good. This was because:

• All Patients had a mental health risk assessment completed on or soon after admission
• Staff update care plans regularly and after significant events
• Staff delivered care in line with national guidance

However:

• Patients on the forensic and secure wards were not always involved in developing their care plans
