During an inspection looking at part of the service

At our last inspection we found there were not effective systems in place to reduce the risk and spread of infection and that improvement was needed. During this visit we found that vast improvements had been made and that people were protected from the risk of cross infection.

People were protected against the risks associated with medicines because the provider had put in place robust arrangements to manage medicines

The registered provider of the home has continued to make improvements to the fabric of the home. All areas of the home were clean and in good repair.

Systems and processes were in place to monitor the quality of care that people receive.

During an inspection looking at part of the service

During a routine inspection

We (the Care Quality Commission) carried out this inspection as part of our planned review of inspections.

We talked with four out of the nine people who lived at the home and four people who received care in their own homes. They told us that they were happy with the way they were cared for by staff from Fernleigh. Their comments included 'Staff are lovely, they look after me like a mother would' and 'staff are absolutely smashing, they arrive on the dot every time'.

People told us that at all times staff treated them kindly and respectfully. They said that in the home they had choices about what they ate and how the spent their time. Everybody we spoke with told us that they could talk to the registered manager if they had any problems and felt they would be resolved quickly and appropriately.

The registered provider of the home has continued to make improvements to the fabric of the home. However, not all areas of the home were clean or in good repair.

People were not protected against the risks associated with medicines because the provider had not put in place appropriate arrangements in place to manage medicines. This is for the regulated activity of accommodation for people who required nursing or personal care and not for the people recieving care in their homes.

The systems in place for monitoring the quality and risks associated with the service were not yet established in all areas to ensure that people were protected against the risks of inappropriate or unsafe care.

During a routine inspection

We talked with five people who lived at the home. They told us 'I am looked after very well - its brilliant' and 'we all get on well, we are happy together'.

People said staff always asked for their consent before providing care and they were happy with the care they had received. We saw that the care plans in place for each person were detailed, personal and reflected each person's choices and preferences. Some records were not signed by staff or completed correctly and this may mean that they would not provide an accurate record of care given.

Medication systems were in place to ensure that people received medication when it was prescribed for them. Some aspects of the management of medication needed to be improved to ensure the safety of people using the service.

We spoke with six staff who told us that in the home and the service provided in the community, they were supported to provide a good level of care. People using the service and staff felt there were sufficient staff on duty to meet their needs. Some training was needed to be updated to ensure current good practice was maintained.

Some auditing systems were undertaken by the management of the home, not all aspects of service delivery were monitored and so some areas had not been improved to a good standard such as suitability of the premises.

Some parts of the home were not clean and the management of risk of cross infection in the home did not protect people.

During an inspection in response to concerns

We carried out a visit on 31 March 2012 and 16 April 2012 (in order to meet with the registered provider). We observed how people were being cared for, talked with people who used services, talked with staff, checked the provider's records, and looked at records of people who used services.

We spoke to five people living at Fernleigh House and staff. There were no issues raised by anyone, all were very positive about the care provided and the staff providing the care. Some people who used the service were independently moving around the home and staff were seen to interact well with them.

People we spoke with confirmed that they chose how to spend their days, and that they could do what they liked.

We saw that people were spoken to in a respectful and caring way. People told us that the staff were 'lovely' and that 'they will do anything for you'.

People told us they were happy with the care and support they received at Fernleigh House.

People said that the staff were 'very kind' and spoke very highly of the staff group. People told us about the activities that they enjoyed and the things they liked to do.

We saw that people's rooms were personalised with their own furniture, keepsakes, pictures and photographs.

We saw that people who use the service were happy to approach staff. People told us that they could discuss anything with any of the staff.

Senior staff told us they would feel able to raise an alert for any perceived abuse. The staff we spoke to were aware of the role of the external agencies in safeguarding, such as Social Services.

Two people we spoke with told us that they thought there were enough staff. They said they did not have to wait very long to have their call-bell answered if they rang it.

The three staff members we spoke all said that they thought that there were enough staff for the number of people they were looking after.

The three staff we spoke to said they enjoyed working at Fernleigh House. They said they were free to discuss anything with the registered manager/provider and senior staff.
