During a routine inspection

There were 20 people living at St Helens House at the time of this inspection visit.

We reviewed the recruitment practice and associated documentation. We found the home followed robust recruitment processes for the recruitment of new staff.

During a routine inspection

There were 15 people living at St Helen's House at the time of this inspection visit. We spoke to six people who used the service and a visitor. We also spoke with four staff members including the manager and were able to meet with the registered provider. Some people had complex needs and were not always able to verbalise their experiences, so we observed the interaction between staff and people closely.

We saw staff asked for people's consent before they provided any care or treatment. People, who were able to, told us they were asked for their consent before any care and support was provided. One person told us 'I could not be looked after better, I am very happy with the way the staff look after me. I have a bath when I want but they do not say I have to have one' Another told us that they were involved in writing their plan of care and agreed to the care provided.'

We looked at systems in place in relation to the cleanliness and infection control. The standard of cleanliness and hygiene had improved.

We saw that people received their medicines as prescribed and systems were in place to protect people against the risk associated with medicines management.

We reviewed the recruitment practice and staffing provision we found the home did not follow robust recruitment processes but there were suitable staffing arrangements in place to meet the needs of people with the current occupancy.

During an inspection in response to concerns

The Care Quality Commission received information of concern in relation to the recruitment practice followed, the staffing numbers, and the skills of the staff working in the home. We visited the home on two occasions in December 2012 to follow up on a compliance action made in May 2012, and to review the staff recruitment and deployment practice in the home.

We looked at systems in place in relation to the cleanliness and infection control. The standard of cleanliness and hygiene had been improved since the last inspection visit completed in May 2012. However, systems to maintain and monitor good standards of cleanliness and infection control were not established.

We reviewed the recruitment practice and found some concerns about the process. We looked at the staffing arrangements and found that they were not based on the assessed needs of people who used the service. Although staff and people told us that the staffing arrangements were safe, there was evidence that they did not meet the needs of people.

During an inspection in response to concerns

People spoken with told us that they felt that they had their privacy and dignity respected. People told us that were given choices and consulted on the care and support provided.

People using the service spoken with expressed a satisfaction with the care and support provided.

People's comments included 'I am treated very well'. 'I like it here'.

A visiting health care professional spoken with following the inspection visit, confirmed that the home worked well with them, and responded appropriately to their advice and guidelines.

People told us that they got their medicines when they should and when they needed them.

All feedback about the staff was positive with comments including 'the staff are lovely', 'the staff are very nice', 'the staff are kind to me'.

Everyone spoken with said that if they had any concerns they could talk to the staff and the owner. They felt that they would be listened to and their concerns would be responded to.

During an inspection in response to concerns

One person told us 'We have games and bingo and a hairdresser comes in once a fortnight. I come down for breakfast about 8am, and everyone tends to be upstairs by about 9pm. We get to go out for trips, sometimes into the country. The staff would listen to me (if I expressed a choice)'. Another person said 'I have my breakfast brought up. I can't fault the place. I couldn't be any happier'.

Someone told us 'The food is really excellent. It's home cooked and absolutely delicious.'

Someone told us,'I haven't really seen my care plan. Another said 'I've never seen my care plan.'

Someone told us 'I feel safe here. I like the security of here ' I'm quite happy as I am'.

One person said 'They put the (medication) pot in front of me and let me take it whenever'. Another said 'Staff leave it (medication) with me ' that's how I like it. They don't need to watch me.'
