This is the first inspection for this service. At this inspection this key question has been rated good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care,wellbeingand communication needs with them.Care plans included detailed medical histories, personal preferences, and risk factors. Assessments considered physical health, mental capacity, and social needs, with input from family members and professionals. This helped ensure care was tailored and responsive to changing circumstances.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards .Plans included appropriate interventions for conditions such as dementia, chronic kidney disease, and mobility issues. Staff followed structured routines and used verbal prompts and visual aids to support independence and reduce risk.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. There was clear evidence of joint working, such as involving legal representatives in decision-making and liaising with pharmacists around medication safety. Feedback from relatives confirmed that communication was effective and that staff were responsive to concerns.

 

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence ,choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. Care plans included goals around nutrition, hydration, mobility, and social engagement. Staff encouraged participation in community activities and hobbies, and supported people to make healthy choices. One person expressed interest in attending local groups, and information was shared with their family to explore this further.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. Care plans were updated to reflect changes in health and capacity. There was evidence of improvement in mobility and independence for some individuals. Feedback indicated that people felt more confident and supported in their daily lives.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Staff assessed capacity for each decision and deferred non-urgent care when consent was unclear. Where people lacked capacity, best interest decisions were made with involvement from family members and legal representatives. Copies of legal documents were viewed and referenced in care plans. We queried how people deemed not to have capacity signed their care plans, as this was sometimes unclear. The provider agreed to clarify this within care plans.
