Our current view of the service

Requires improvement

Date of assessment: 20 to 28 January 2026. This service is a care home providing personal and nursing care for up to 36 older people, some who may be living with physical disabilities. At the time of our assessment 29 people lived at Brookfields Private Nursing Home.

This assessment was completed due to the length of time since the last ratings inspection, and in part, due to partner agencies finding that improvements were needed in areas such as medicines management, record keeping, care planning, governance and oversight.

We found 1 breach of the legal regulations in relation to governance systems.

Accidents and incidents were reported but not always reviewed in a timely manner and sometimes, not all expected steps had been taken in response. Some improvements to care plans and risk assessments were still needed and were being worked on by leaders at the service. Not all risks in the environment had been identified and mitigated, and improvements were needed as to where personal protective equipment (PPE) was stored. Some improvements had been made to medicines management; however, we found not all medicines had been stored safely.

Improvements were needed in some care records to show how people’s health conditions were being monitored. Improvements were needed to the governance systems in place at the service to help ensure any shortfalls could be identified, risks reduced and improvements made.

The provider completed checks on staffs’ suitability for the job role and supported their staff team well. Staff worked well together and communicated with each other effectively. They knew people’s care needs and were kind, caring and respectful. They provided personalised care and promoted people’s independence, choices and control.

Activities were planned to ensure they offered support to people’s well-being. This helped people stay connected to their local community and provided opportunities to link to local schools and local community facilities. Staff understood the value of people taking part in special events and worked hard to make participation in significant events possible. Activities helped to link people to seasonal events as well as cultural and religious festivals.

Staff were well supported by leaders in the service. Leaders understood how to help staff with changes to their work schedule when needed. Staff felt able to speak up about any concerns and were confident these would be dealt with appropriately be leaders.

Leaders worked to create a positive and inclusive work environment for staff. They worked to provide personalised care for people. Leaders were responsive and keen to make improvements in the service. They worked well in partnership with others to achieve good outcomes for people.

We have asked the provider for an action plan in response to the concerns found at this assessment.

People's experience of the service

People were welcomed and helped to settle in when they moved to Brookfields Private Nursing Home. They felt safe and well-cared for and felt staff knew them and their care needs well. They felt there were enough staff and praised their kindness and the homely atmosphere they helped to create. One person told us, “I would recommend it here. It’s a real home from home and everyone’s kindness means a lot.” People were happy with how their home was kept clean and well-maintained and were happy with their meal and drink choices. People received support from a range of health care services when needed and they were happy with how their medicines were managed by staff.

People found staff were kind and respectful and checked they consented to any care and treatment. They received care that was personalised to them and reflected any specific needs such as any religious observances. They found staff were able to respond to any immediate needs, for example, if they needed any pain relief.

People were supported to participate in activities that were beneficial to them and fostered links to the community and promoted a good rapport between people and the staff team. They were supported by staff that recognised the value of people taking part in and celebrating significant life events.

People’s views were listened to and understood by leaders and staff. If any issues were raised, leaders worked to resolve these for people and included them in how they did so.
