During an assessment of Diagnostic imaging

This was the first assessment of the service since registration in January 2022.

We found a breach of Regulation 17: Good governance relating to safeguarding, consent processes and infection prevention and control.

We identified significant shortfalls in governance arrangements. The provider could not demonstrate effective oversight of quality, safety and performance. Governance systems had not ensured policies were current. Staff training requirements were not clearly defined. Incidents were not reviewed effectively, and audits were not undertaken to provide assurance about the quality of care being delivered. Staff did not always have the right safeguarding training; consent processes were inconsistently applied and documented; and infection prevention and control risks were not adequately assessed or managed.

However, we found women were treated with kindness and compassion and spoke positively about their experiences of care. Staff worked effectively with external healthcare providers to ensure continuity of care when concerns were identified or referrals were required.

During an assessment of the hospital overall

The Women’s Scan Clinic is an independent pregnancy scanning service which provides diagnostic scans at all stages of pregnancy. Pregnancy scanning is completed by sonographers (trained health workers who use ultrasound technology) who have received training in various aspects of fetal wellbeing.

Throughout this report we refer to pregnant women and people or service users as ‘women.’ This was the first assessment of this location since it was registered in January 2022.

In our assessment of The Women’s Scan Clinic, we found systems and processes for governance were not properly established. We identified a breach of Regulation 17: Good governance. The service did not effectively assess and monitor the service to mitigate the risks to health, safety and welfare of women using the service.

However, the staff were caring and gave women the time and attention they needed during appointments. One woman said they “had a great service and another felt “very reassured.”
