During a routine inspection

Flora Lodge provides care and support for twelve people with mental health needs and has a registered manager in post. We met eight people who used the service during our inspection and asked them about their experiences of the service.

We found people were protected from the risk of abuse as effective systems were in place and staff understood what action to take to protect people. However the behaviour of some staff at times was not respectful of people’s sensibilities, emotional well-being and dignity. Although everybody we spoke with felt safe living at the home, we were concerned that the attitude and behaviour of some staff may have at times undermined this. .

There was also limited understanding and implementation of the Mental Capacity Act and Deprivation of Liberty Safeguards. Risk assessments were in place but we found them to be inadequate. People received their medication as prescribed but overall the management of medicines did not protect people from the associated risks.

There were systems in place that monitored the quality of service and the delivery of care and support however, this had not been fully effective in identifying concerns with risk assessments, care planning or staff interactions with people and had therefore not lead to the necessary improvements. The service had not met CQC requirements to notify us about all the events they are required to inform us about by law.

Care plans recorded people’s wishes and preferences but did not always provide detail about how staff should meet people’s needs. We found that people’s health had been monitored and guidance from health professionals had been sought when appropriate.

We spoke with staff and found they had received appropriate training, supervision and support to enable them to provide effective care to people. There were sufficient numbers of staff to ensure the safe and effective delivery of care. We observed the staff team treating people with kindness and the manager, in particular, had developed good relationships with the people who used the service.  Comments from people living in the home included: “I love it here”, “The food here is good,” and “The manager is very approachable”.

Records showed that people’s views, wishes, preferences and concerns were sought, listened and responded to.

We found a number of breaches of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010. You can see what action we told the provider to take at the back of the full version of this report.

During an inspection looking at part of the service

We spoke with two people who used the service and explained to them why we had returned to Flora Lodge. We told them that when we had visited earlier in the year that we had been concerned that people may not be protected from the risk of abuse. The two people we spoke with told us that they had attended a residents meeting and that they had spoken about the different types of abuse and what they should do if they were being abused. They also told us that they had been given written information about the different types of abuse which included contact details so they could report abuse if they didn't want to speak with staff at Flora Lodge, both people told us they had kept the information in their bedroom.

During a routine inspection

We spoke with three people who used the service on the day of our visit. People told us they were happy living at the home and did not wish to leave. People went onto tell us how they accessed the local community either independently or with staff support. One person showed us what they had bought that morning from their trip into town, whilst others spoke of a day trip to Skegness. People told us about their health care needs and how staff supported them in managing their mental health by being approachable to speak with.

We saw people coming and going from Flora Lodge and taking part in daily living activities within the home. We saw one person sweeping the leaves up in the courtyard. Later on we saw the person playing 'ball' with a dog which they clearly enjoyed. People's comments included: 'I'm very happy here. I have my say about what's important to me and I get to socialise with my friends.' 'I like living here, I'm looked after very well and I am able to come and go as I please.' 'I've lived here many years. The staff are helpful and they are there if I become worried or upset about something.'

People were not sufficiently protected from the possibility of abuse as the service was not proactive in referring potential safeguarding concerns to the local authority. Improvement is needed in this area.

Records were found to be accurate and fit for purpose.

During an inspection looking at part of the service

We did not speak with people using the service as a majority of the work to improve the environment was still taking place. We will ask people for their views when we next inspect the service.

We found a number of improvements to people's bedrooms had taken place. We found that improvements continued to take place at the time of our visit and the provider told us about proposed future plans.

During a routine inspection

People's views and experiences were taken into account when planning and delivering people's care and support. Resident meetings were held to discuss general issues which included menu planning and day trips. We spoke with two people who when we asked about resident meetings told us 'they then get in what we have asked for.' 'We all get to say where we want to go.' 'You get a chance to say your views'

People when asked about their views of the service told us: - 'It's the best place I've lived.' and 'you can ask any time of the day if you need something and they get it for you.'

People who use the service told us staff were helpful and that they could talk to them. We saw people throughout our inspection approach the registered manager and staff members seeking reassurance about aspects of their daily lives.
