During a routine inspection

Mellor Nook is a 'care home'. People in care homes receive accommodation and nursing or personal care as a single package under one contractual agreement. We regulate both the premises and the care provided, and both were looked at during this inspection.

Mellor Nook is located in the small town of Mellor in Stockport. The home provides care and accommodation for up to 15 older people. Bedrooms are situated on the ground floor and first floor of the home. Access between floors is via a stair lift and staircase. Eleven bedrooms have an en-suite toilet. The building is situated in its own grounds with secure gardens and off-road parking. At the time of our inspection 13 people were living at Mellor Nook Residential Care Home.

This unannounced inspection was conducted by one adult social care inspector from the Care Quality Commission on the 24 August 2018.

At our last inspection in November 2015 we rated the service good. At this inspection we found the evidence continued to support the rating of good and there was no evidence or information from our inspection and ongoing monitoring that demonstrated serious risks or concerns. This inspection report is written in a shorter format because our overall rating of the service has not changed since our last inspection

The provider is required to have a registered manager as part of their conditions of registration. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are 'registered persons'. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run. At the time of our inspection, the service was in the process of appointing a new registered manager.

People said staff treated them with kindness and compassion. Comments included, "They are very kind, I feel safe and cared for," and "They are kind, considerate and available if you need assistance."

Staff knew people's care needs, preferences, personal histories and backgrounds. People said staff protected their privacy and their dignity was respected. People were supported to be independent.

People received care from staff who were appropriately trained to effectively carry out their job roles. People were supported to have maximum choice and control of their lives. The service acted in accordance with the Mental Capacity Act (2005). People's nutritional needs were met and they were supported to maintain good health and receive ongoing healthcare support.

Staff were aware of their responsibilities to safeguard people from abuse. Safe recruitment practices were in place and the service followed national and local safeguarding guidance. There were sufficient staff to care for people. Risks to people's safety were assessed and medicines were administered safely.

People received personalised care that met their individual needs. People were given appropriate support and encouragement to access meaningful activities and follow their individual interests.

We found that records written in a positive and respectful way we found that records provided guidance on how to support people.

Mellor Nook had arrangements in place to receive feedback from people that used the service, their relatives, external stakeholders and staff members about the services provided.

We found that records were sufficiently maintained and effective systems in place to monitor the quality of the service.

During a routine inspection

This was an unannounced inspection of this location. This inspection took place on 19 and 23 November 2015.

The service was previously inspected on 15 January 2014 when no breaches of legal requirements were found.

Some staff we spoke with were not confident about their duties and responsibilities in relation to the Mental Capacity Act (MCA) 2005 and Deprivation of Liberty Safeguards (DoLS) even though they had received training in this topic. The deputy manager advised us that she would arrange further staff training to help make sure their confidence and knowledge was increased in this area.

People who used the service, who we asked, told us that Mellor Nook was a safe place to live and felt they were well looked after.

Staff understood their role in making sure they safeguarded vulnerable people from harm and had undertaken training in adult safeguarding.

Care plans highlighted in detail the areas of support people needed and included associated risk assessments. Medicines were managed safely.

People were supported by sufficient numbers of suitably trained staff, who had been appropriately and safely recruited to support and meet people’s individual needs.

Care staff had all received a thorough induction, training and support when they started work at the home and understood their roles and responsibilities, as well as the values and philosophy of the home. Staff had a clear understanding of the care and support people required and people we saw looked well cared for and comfortable in their surroundings.

The staff training record showed staff had access to a range of appropriate training such as dementia awareness, pressure care awareness and end of life care and the staff we spoke with confirmed this. They also told us that they felt well supported by the manager and found the management team very approachable.

People were provided with personalised care by staff who supported them to live as independently as possible. People were also supported by staff to eat and drink enough to maintain a balanced diet. Staff knew how to make sure the care provided followed best practice as detailed in the care plans. We found that people’s care was delivered consistently by care staff who knew how to support people and meet their assessed care needs.

People spoken with told us that the staff were caring and we observed good relationships between individual staff and people who used the service. We saw that care was provided with kindness; staff were respectful when speaking with people and responded promptly when people required assistance.

We saw people who used the service were engaged in meaningful activity to promote their wellbeing and prevent social isolation People told us they knew who to speak to if they wanted to raise a concern or complaint. A copy of the complaints process was displayed in prominent areas throughout the home.

To help make sure that people received safe and effective care, systems had been put in place to monitor the quality of service being provided. These systems included regular checks on all aspects of the management of the service.

We saw that the cleaning system in place helped to make sure the home was clean and there were no offensive odours apparent during our visit.

15 January 2014

During a routine inspection

People spoken with told us their views and experiences were taken into account in the way the service was provided and delivered.

We found that the care records identified people's needs in relation to the care and support they required to maintain their health and wellbeing. Two people spoken with indicated that the staff provided sensitive and flexible care and support and they were well cared for.

We observed staff administering medicines and saw that these were administered safely.

We saw that people were living in safe, accessible surroundings that promoted their wellbeing.

We looked at the staff training records and noted that staff had been provided with training relevant to their job role.

We saw that records required to be kept to protect people's safety and wellbeing were maintained and held securely where required.

15 January 2013

During a routine inspection

People told us what it was like to live in Mellor Nook and how the staff cared for them and supported their needs. One person told us, 'The staff are excellent, you cannot fault them, the support they offer you is wonderful. I know I couldn't manage without them.' Another person said, 'I couldn't complain about any of the staff here, they are all very good'.

We found that each person using the service had a care plan in place and from speaking with the managers and staff it was demonstrated that they had a good knowledge and understanding of people's needs and how best to meet those needs.

We found evidence that staff received training but this had been inconsistent and improvements were needed. We have asked the provider to take appropriate action to address this matter.
