The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity.

People received consistent support from a stable staff team who knew them well. One relative told us the whole staff team worked well and if their family member needed support from external professionals this was always arranged.

Staff worked with external professionals to ensure care was well-coordinated and continuous. One health professional told us staff were proactive in referring medical problems in a timely manner. Another told us, “They [staff] are proactive in their approach to contacting us early with any health deterioration or wellbeing concerns. I have no concerns with how well they managed this.”
