Assessing needs

Score: 4

Assessments identified achievable outcomes and were regularly monitored to measure progress and adjust support where required. This collaborative approach meant people consistently received high quality, personalised care that promoted their wellbeing, choice and independence.

Assessments were comprehensive and person-centred, taking full account of the individual's needs alongside the impact of their environment.
