Well-led
Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.
At our last assessment we rated this key question as good. At this assessment the rating has remained as good.
Leaders promoted a positive and open culture and staff spoke positively about the support they received. Whilst some opportunities were identified to strengthen aspects of record keeping and oversight, leaders were receptive to feedback, demonstrated a commitment to improvement and continued to promote person-centred care.

The provider had a shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people and their communities.

Governance, management and sustainability
The provider had governance arrangements in place; however, these were not always effective in identifying and addressing areas for improvement. Care plans and associated records did not always provide enough detail to guide staff practice. Management oversight of records, daily notes and incidents was not always robust enough to consistently demonstrate analysis and learning. Whilst leaders responded positively to feedback and made changes during the assessment, further improvements were needed to strengthen governance and oversight arrangements.

Learning, improvement and innovation
The provider demonstrated a commitment to continuous learning, improvement and service development. Leaders welcomed feedback and undertook reviews where areas for improvement were identified, fostering a culture where staff felt able to raise concerns and share ideas. However, quality assurance processes had not always been effective in identifying and addressing issues at the earliest opportunity, and greater oversight was needed in some areas to ensure improvements were embedded and sustained.
