During an inspection looking at part of the service

People were given a choice of activities and were supported to be as independent as possible. A person we spoke with told us that staff assisted them to tidy their room. Relatives that we spoke with views on communication from the home varied, between being kept informed of their relative wellbeing 'all the time' to 'not being told kept informed about their relatives medical appointments'. Relatives that we spoke with felt that the high turnover of managers had some impact on the continuity of care for their relatives but felt that recently the service had improved.

Each person had a care plan which included details on how they wanted staff to support them. Care plans included a 'missing person' Performa which could be handed to the police should a person go missing. Individual risk assessments had been undertaken but we found that one person's risk assessment did not contain vital information to ensure that the person and other people were kept safe. This was addressed by the home on the day of our inspection.

The home had its own safeguarding policy and procedure in place and also had a copy of Hertfordshire safeguarding vulnerable adults 'making a referral' document. This provided details of how staff could make a referral to the local authority safeguarding team.

The home had an appropriate recruitment process in place and checks had been made to ensure that staff were suitable and qualified for the post. We saw evidence that staff had received mandatory training, but staff did not always receive supervision and did not receive a yearly appraisal. Staff we spoke with felt that there was enough staff to meet the needs of the people who resided in the home.

The provider had undertaken health and safety checks but had not always responded to the findings to ensure that people were residing in a safe environment. There was not an appropriate system in place to obtain the views of relatives and stakeholders.

During an inspection in response to concerns

People we spoke with were happy with the service they had received from the home. We saw that staff interacted in a positive way with people.

We found that people's activities in the community were sometimes restricted.

Risk assessments did not always provide staff with adequate information on how to deal with the risk.

Some staff we spoke with did not understand the concept of safeguarding vulnerable adults and the home's policy and procedure was not readily available for staff to refer to.

The home did not have effective and robust processes and procedures in place which ensured that staff members were suitably employees.

Staff shortages meant that the manager was required to cover staff when people who used the service were assisted by staff to undertake activities in the community.

The home did not have sufficient number of staff to meet the needs of the service users.

Staff had not been given regular supervision to discuss their progress and any concerns and not all staff had received the mandatory training that they needed to carry out their role.

The home did not have effective systems in place to regularly assess and monitor the quality of the service that people received.

The home did not have an effective complaints system available.

During an inspection looking at part of the service

Most of the people who live at Abbey Lodge had limited communication skills and therefore we were unable to get their views of what it is like to live at Abbey Lodge.

One of the people we spoke with told us that they liked living at Abbey Lodge and that they could go to work once a week at the hairdressers where they liked working. Relatives of the people told us that they were very happy with the care. They said that their relative was progressing well in the home and was starting to have a social life outside the home. We were told that this was very good progress. They said that they can leave the home without any worries and know that their relative was being well cared for. We were told that the staff were kind and caring and that the home was improving daily.

Visiting professionals told us that the home had improved and that the intervention plans they had drawn up to improve the care of people had been implemented and as a result the quality of the lives of the people had improved

During an inspection in response to concerns

When we visited Abbey Lodge on 13 February 2012 we were able to have brief conversations with some of the people who live there, we were also able to see the way that care and support were provided for them and the interaction between them and care staff. All of the interactions that we saw were appropriate and respectful and included conversations about choices for example what to have for breakfast and where they would like to go that morning.

One person told us that they had been playing snowballs a few days earlier and that they knew what they would be doing tomorrow because it was Tuesday.

None of the people who live in Abbey Lodge demonstrated either by verbal or non verbal communication that they were other than well cared for and throughout our visit the atmosphere in the house was reasonably calm and stress-free.

On this occasion we did not speak to people who live in Abbey Lodge about safeguarding people who use services from abuse, staffing, assessing and monitoring the quality of service provision or records.
