During a routine inspection

We carried out this inspection under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. This inspection was planned to check whether the provider is meeting the legal requirements and regulations associated with the Health and Social Care Act 2008 and to pilot a new inspection process being introduced by the Care Quality Commission (CQC) which looks at the overall quality of the service.

This was an unannounced inspection.

The service met all of the regulations we inspected against at our last inspection on 15 July 2013.

Worcester Road provides personal care, support and accommodation for up to seven people with learning disabilities. At the time of our visit there were six people using the service.

The service had a registered manager in place. A registered manager is a person who has registered with the CQC to manage the service and has the legal responsibility for meeting the requirements of the law; as does the provider. People using the service told us they felt happy living at Worcester Road and they liked the staff who supported them. We saw that support plans and risk assessments were reviewed every six months by staff and each person using the service.

We saw staff had been trained in the requirements of the Mental Capacity Act 2005 (MCA) and they had a good understanding of the principles of the Act. There were systems and processes in place to protect people from foreseeable harm and act on concerns in order to keep them safe. We found there were no Deprivation of Liberty Safeguards (DoLS) in place at the time of our inspection.

We saw the service had clear and detailed recruitment procedures that involved people using the service as part of the interview process.

The bathroom and shower room were in need of redecoration and deep cleaning. The kitchen was in need of refurbishment as it had damaged drawers and cupboards. The manager informed us a refurbishment was scheduled for September 2014.

Staff completed a range of training defined as mandatory by the provider. The staff we spoke with felt they had the appropriate training and support to carry out their role.

Staff explained how they promoted and maintained people's privacy and dignity through their support they provided. We saw that staff treated people in a caring manner with dignity and respect.

We saw the support plans included information on how to resolve any behaviour that could be challenging and the activities people took part in each week.

People using the service and their relatives were sent annual questionnaires to gain feedback on the care and support provided. Information from incident and accident reports was used to identify any changes in need and ensure the support plans were reviewed to reflect this. People using the service were involved in decisions about how the service was provided and had been consulted on possible major changes to the service.

During a routine inspection

We looked at the care records of two people, spoke with four people who use the service, two members of staff and the manager. People spoke positively about the service. One person told us " I feel safe here, they help me to do things I like." Another person told us "I like to go out and visit my relative, I get help to do that." People told us about their interests and experiences of using the service and told us they were encouraged to be as independent as possible by engaging in activities such as cooking, washing and going out into the community to do shopping tasks.

We carried out a visit on the 12 December 2013 where we found improvements were needed in the planning and delivery of people's care. We found during this visit that people had detailed care plans that were reviewed on a annual basis o ensure their needs were being met. People were supported to attend health appointments such as the dentist and GP when there was a need or for annual check up's to ensure they were maintaining good health.

We also found at our visit on the 12 December 2013that the physical environment of the home was in need of repair. During this visit improvements had been made. We saw that, areas of the service had been re-decorated and were being adequately maintained.

At our visit on 12 December 2013 staffing levels at the service were inadequate to meet people's needs. During this visit we found that improvements had been made and staffing levels had been increased to meet people's needs.

We looked at records relating to people's care and found they were kept securely and were adequately maintained to ensure the information relating to people was up to date. This meant people were protected against the risks that can arise if records about their care were not kept appropriately.

During a routine inspection

During our visit we spoke with four people who use the service and two staff. We also looked at records regarding feedback received from people and their relatives.

People who use the service told us they liked living at the home and enjoyed the food provided. People said the staff were friendly and knew what they liked. We saw staff being kind and respectful towards people. They communicated with each person in different ways, depending upon their needs.

However, we found that people were at risk of inappropriate care and people's needs were not always able to be met. Some people were not able to go out when they wanted and we found one person had not been out for weeks at a time. There were not enough staff to meet the needs of the people who use the service. The environment of the home was not adequately maintained, where some areas could put people at risk.

During a routine inspection

People who use the service told us that they enjoy living at the home, that staff are available when they need them and that they get the support they need. People said that they usually enjoy the food provided at the home and that they can choose what they eat.

People told us that they feel safe living at the home and that they would feel comfortable speaking to staff if they were unhappy about something. People said that they can choose how their rooms are arranged and decorated and that the home is kept fresh and clean.
