Shared direction and culture

Score: 4

The provider had a very clear shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and an exceptional understanding of the challenges and the needs of people and their communities.

The service demonstrated an exceptionally strong, values-driven culture that was consistently understood, embraced and promoted at all levels of the organisation. A shared sense of purpose united leaders, managers and staff, ensuring people remained at the centre of decision-making and service delivery. This culture was underpinned by MacIntyre's DNA, the provider's value-based approach to support. The DNA was deeply embedded throughout the service and was evident in the attitudes, behaviours and practices of staff, influencing how they interacted with people, supported positive outcomes and responded to challenges.

The provider's values were consistently reflected in people's experiences and outcomes. Decisions were guided by what was important to people, and staff worked collaboratively to ensure support remained person-centred, ambitious and focused on achieving meaningful goals. As a result, the service had developed a positive and empowering culture where people, families and staff shared a common understanding of the provider's vision, creating an environment that consistently drove high-quality care, innovation and exceptional outcomes.

Governance, management and sustainability

Score: 2

The provider did not always have clear systems of accountability or good governance arrangements. They did not always act on the best information about risk, or share this with others when appropriate.

The provider had a range of governance and quality assurance systems in place to monitor the quality and safety of the service. However, these were not always implemented consistently or operating as effectively as intended across all levels of leadership. We found that some monitoring processes designed to identify and address areas of risk had not been completed consistently, which reduced opportunities to provide timely assurance regarding the quality and safety of service delivery.

In addition, overarching governance arrangements were not always updated in a timely way to ensure the registered manager had a complete and up-to-date overview of concerns identified across the service. This limited the provider's ability to demonstrate effective oversight of emerging themes and the actions taken in response. Whilst leaders showed a commitment to continuous improvement and took action when issues were identified, the governance framework did not always support the prompt escalation, analysis and review of information. As a result, there were missed opportunities to strengthen oversight, evidence the effectiveness of actions taken, reduce the likelihood of repeated incidents and provide consistent assurance that quality monitoring systems were driving sustained improvements across the service.

In response to the concerns identified during the inspection, the provider took immediate action to strengthen governance and oversight arrangements. Leaders reviewed existing processes, introduced updated procedures and implemented additional monitoring systems to improve the identification, escalation and oversight of concerns.
