At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing, and communication needs with them; the person told us this happened and records confirmed this process. Staff kept people’s needs under assessment and worked with families and external agencies when changes occurred. We saw staff consulted with the person to identify if their needs were being met or if any changes were needed.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. The person’s support was designed to meet their individual needs and to deliver a good quality of life. The person was supported to pursue a lifestyle specifically designed to support their personal interests and specific needs. Some activity involved some risk, but the service had recognised the person’s right to choose and had mitigated these risks as far as possible.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Staff worked collaboratively with other professionals to ensure the person received coordinated and safe care. Records showed where staff had concerns, they escalated this to appropriate health or social care agencies. The person told us the staff team worked well together to ensure they had the support they needed.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and, where possible, reduce their future needs for care and support. The person described how staff supported them to stay healthy, including making sure they had enough to eat and drink and followed a balanced diet. Staff supported the person to access healthcare to ensure their health was monitored and maintained.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. The person told us they were happy with the care they received. They told us, “Having the staff here has made all the difference to me. They help me with the things I can’t do anymore.”

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. The provider had Mental Capacity Act 2005 policy and procedures in place and staff had received training in these. This helped staff to understand their roles and responsibilities in protecting people’s rights and capacity to make decisions and choices. The person told us they consented to their care and treatment. They confirmed they were offered choices; these were respected and clearly documented in their care plan.
